玻璃体切除联合内界膜剥除及后巩膜加固术治疗变性近视黄斑孔

Vitrectomy combined with internal limiting membrane peeling and posterior scleral reinforcement for macular hole of degenerative myopia

摘要 目的评估玻璃体切除联合内界膜剥除及后巩膜加固术治疗变性近视黄斑孔的效果.方法回顾性系列病例研究.分析2014年2月至2016年8月该院变性近视黄斑孔进行玻璃体切除联合内界膜剥除及后巩膜加固术53例(53眼)的临床资料.结果术前BCVA(Log MAR)为0.78±0.23,术后提高至0.62±0.19(P=0.021).术前眼轴(29.52±4.67)mm,术后缩短为(27.91±3.87)mm(P=0.041).术前屈光度为(-17.50±3.65)D,术后减低为(-14.20±2.84)D,手术前后相比无统计学意义(P=0.004).黄斑孔闭合率为90.6%(48/53).无医源性裂孔、眼内出血或眼内炎等严重并发症发生.结论玻璃体切除联合内界膜剥除及后巩膜加固术治疗变性近视黄斑孔安全有效. Objective To evaluate the efficacy of vitrectomy combined with internal limiting membrane peeling and posterior scleral reinforcement for the treatment of macular hole of degenerative myopia.Methods A retrospective case study.The clinical data of 53 eyes of 53 cases of degenerative myopia with macular hole were treated with vitrectomy combined with internal limiting membrane peeling and posterior scleral reinforcement From Feb.2014 to Aug.2016 in this hospital were analyzed.Results BCVA(Log MAR)was 0.78±0.23 before operation and 0.62±0.19 after operation(P=0.021).Preoperative axial length was(29.52±4.67)mm,and the postoperative length shortened to(27.91±3.87)mm(P=0.041).The preoperative mean refractive status was(-17.50±-3.65)D,and the postoperative status was decreased to(-14.20±-2.84)D(P=0.004).The closure rate of macular hole was 90.6%(48/53).No serious complication such as iatrogenic tear,intraocular hemorrhage or endophthalmitis occurred.Conclusion Vitrectomy combined with internal limiting membrane peeling and posterior scleral reinforcement is safe and effective in the treatment of macular hole of degenerative myopia.
