摘要
目的分析白细胞、C反应蛋白和免疫球蛋白检测在小儿肺炎中的应用价值。方法回顾性分析2018年1月-2019年3月该院42例肺炎患儿的临床资料,其中细菌感染和病毒感染各21例,并收集同期于该院接受健康体检的23名正常儿童为对照组,比较肺炎患儿与正常儿童的白细胞、C反应蛋白和免疫球蛋白水平。结果白细胞计数、C反应蛋白水平对比,细菌组与病毒组明显高于对照组,细菌组高于病毒组,差异有统计学意义(F=20.587、18.645,P=0.000、0.000);IgA、IgG、IgM各项免疫球蛋白水平对比,细菌组与病毒组低于对照组,细菌组IgA=(0.54±0.12)g/L、IgG=(5.51±1.22)g/L、IgM=(0.53±0.15)g/L,低于病毒组,差异有统计学意义(F=25.917、13.244、16.033,P=0.000、0.000、0.000)。结论在小儿肺炎的临床诊断中,检测患儿白细胞计数、C反应蛋白和免疫球蛋白等项目方便、快速,可为临床判断提供更多参考依据,其中C反应蛋白可辨别病毒感染与细菌感染,而免疫球蛋白检测可对患儿的免疫功能进行评估,通过白细胞计数、C反应蛋白和免疫球蛋白联合检测,可提高临床诊断的准确率。
Objective To analyze the application value of leukocyte, C-reactive protein and immunoglobulin detection in children with pneumonia. Methods The clinical data of 42 children with pneumonia in the hospital from January 2018 to March 2019 were retrospectively analyzed. Among them, 21 cases were bacterial infection and viral infection, and 23 normal children who received health examinations in our hospital were collected as the control group, the levels of white blood cells, C-reactive protein and immunoglobulin were compared between children with pneumonia and normal children. Results The comparison of white blood cell count and C-reactive protein level was significantly higher in the bacterial group and the virus group than in the control group, and the bacterial group was higher than the virus group. The difference was statistically significant(F=20.587, 18.645, P=0.000,0.000);IgA, IgG, IgM immunoglobulin levels were compared, the bacterial group and the virus group were lower than the control group, and the bacterial group IgA=(0.54±0.12) g/L, IgG=(5.51±1.22) g/L, IgM=(0.53±0.15) g/L, lower than the virus group, the difference was statistically significant(F=25.917, 13.244, 16.033, P=0.000,0.000,0.000). Conclusion In the clinical diagnosis of pediatric pneumonia, the detection of white blood cell count, C-reactive protein and immunoglobulin in children is convenient and rapid, which can provide more reference for clinical judgment. C-reactive protein can distinguish between viral infection and bacterial infection. Immunoglobulin assay can evaluate the immune function of children, and the combined accuracy of white blood cell count, C-reactive protein and immunoglobulin can improve the accuracy of clinical diagnosis.
