摘要
目的研究桥接系统内固定治疗骨盆、髋臼骨折的临床效果。方法选取2015年11月~2017年11月于我院接受治疗的30例盆骨髋臼骨折患者为研究对象,均采用桥接系统内固定治疗,其中男20例,女10例,年龄19~68岁,平均(39.3±1.34)岁。所有患者均为闭合性、不稳定型骨折。所有患者于伤后0~8d行桥接系统内固定治疗骨盆、髋臼骨折,记录手术切口长度、手术时间、术中出血量及术后骨折愈合时间,术后定期行X线检查,并按Matta标准评估患者骨折复位固定情况,记录骨折愈合时间,按Majeed标准评估肢体功能。结果患者手术切口长度为13.2~20.4cm,平均长度为17.2cm;手术时间为78~116min,平均(89.5±5.2)min;术中出血量为82.46~133.67mL,平均(116.8±10.02)mL。患者术后获5~22个月随访,平均(9.37±1.21)个月。所有患者术中均未新发重要血管、神经损伤。患者术后骨折复位情况评定如下:优18例,良10例,可2例;骨折愈合时间为5.9~10.6周,平均(9.2±0.49)周。结论桥接系统内固定治疗骨盆、髋臼骨折患者骨折复位效果良好,并发症少,具有临床推广应用价值。
Objective To study the clinical effect of internal fixation with bridging system in the treatment of pelvic and acetabular fractures.Methods 30 patients with pelvic and acetabular fractures who were treated with bridging system internal fixation from November 2015 to November 2017 were selected as subjects of study,including 20 males and 10 females,aged 19-68 years,with an average age of(39.3±1.34)years.All patients were closed and unstable fractures.All patients were treated with bridging system internal fixation for pelvic and acetabular fractures after injury in 0-8 days.The length of incision,operation time,intraoperative bleeding and healing time of fracture were recorded.X-ray examination was performed regularly after operation and the reduction and fixation of fracture were evaluated according to Matta standard.The healing time of fracture was recorded and limb function was evaluated according to Majeed standard.Results The length of incision was 13.2-20.4 cm,with the average length of 17.2cm.The operation time was 78-116 min,with the average of(89.5±5.2)min,and the bleeding volume was 82.46-133.67mL,with the average of(116.8±10.02)mL.The patients were followed up for 5 to 22 months,with an average of(9.37±1.21)months.No major vascular or nerve injuries were found in all patients.Postoperative fracture reduction was evaluated as follows:excellent in 18 cases,good in 10 cases and fair in 2 cases;fracture healing time ranged from 5.9 to 10.6 weeks,with an average of(9.2±0.49)weeks.Conclusion Bridging system internal fixation for pelvic and acetabular fractures has good reduction effect and fewer complications,which is worthy clinical application value.
