摘要
目的探究基于跨理论模型的社区康复护理在社区糖尿病高危人群中的应用效果。方法选取狮山社区卫生服务中心体检出的152例高危糖尿病患者,根据随机数字表将患者分为对照组与观察组,各76例,对照组采取社区常规护理,观察组采用基于跨理论模型的社区康复护理。观察两组患者的自我管理能力及血糖控制效果。结果观察组患者经基于跨理论模型的社区康复护理干预后,患者在健康知识、饮食、运动、足部护理及血糖监测五个方面的评分明显提高,均优于对照组(P<0.05),且观察组患者的FBG、2 h FBG、HbA1c、BMI均明显优于对照组(P<0.05)。结论基于跨理论模型的社区康复护理能够有效提高糖尿病高危人群患者疾病认知水平,提升患者自我管理能力,使患者形成健康的生活方式,改善血糖水平,降低糖尿病发生风险。
Objective To explore the application effect of community rehabilitation nursing based on trans-theoretical model in high-risk population of diabetes in community.Methods 152 cases of high-risk diabetes mellitus were divided into control group and observation group according to the random number table.76 cases in each group.The control group was given routine community nursing,and the observation group was given community rehabilitation nursing based on trans-theoretical model.The self-management ability and blood glucose control effect of the two groups were observed.Results The scores of health knowledge,diet,exercise,foot care and blood glucose monitoring in the observation group were significantly higher than those in the control group(P<0.05),and the FBG,2 h FBG,HbA1c and BMI in the observation group were also significantly better than those in the control group(P<0.05).Conclusion Community rehabilitation nursing based on trans-theoretical model can effectively improve the disease cognition level of high-risk group of diabetes mellitus patients,enhance the self-management ability of patients,make patients form a healthy lifestyle,improve blood sugar level,reduce the risk of diabetes.
