摘要
目的分析甲状腺转录因子-1(TTF-1)、CD56、P40蛋白在肺癌患者中的表达及临床特征与小细胞肺癌预后的相关性。方法选取2015年1月至2019年1月我院收治的小细胞肺癌患者50例均行手术治疗,且保存病理组织。观察比较不同临床特征小细胞肺癌患者的TTF-1、CD56、P40蛋白阳性比率。通过Cox风险模型来分析影响小细胞肺癌患者预后的独立因素。结果50例小细胞肺癌患者的免疫组织化学检测结果提示,TTF-1阳性率为37例(74%),CD56阳性率为44例(88%),P40阳性率为4例(8%)。Cox风险模型分析结果提示,TTF-1是影响小细胞肺癌患者预后的独立因素(P<0.05);CD56及P40的阳性表达情况跟小细胞肺癌患者的预后没有相关性(P>0.05);除了TTF-1之外,影响小细胞肺癌患者预后的独立因素还包括:吸烟指数、美国东部肿瘤协作组(ECOG)评分及是否为上腔静脉综合征(P<0.05)。结论影响小细胞肺癌患者预后的独立因素为TTF-1、吸烟指数、ECOG评分及是否为上腔静脉综合征;检测TTF-1、CD56及P40的表达情况能够辅助临床对小细胞肺癌进行诊断及鉴别。
Objective To investigate the correlation between the protein expression of TTF-1,CD56 and P40,clinical features and prognosis of small cell lung cancer.Methods Fifty patients with small cell lung cancer admitted to our hospital between January 2015 and January 2019 were included in the study.All patients underwent surgery,and the pathological tissues were obtained.The positive rates of TTF-1,CD56,and P40 protein in patients with different clinical features of small cell lung cancer were determined and compared.The independent factors affecting the prognosis of patients with small cell lung cancer were analyzed by the Cox hazard model.Results The immunohistochemical results of all 50 small cell lung cancer patients showed that the positive rate of TTF-1 was 74%(n=37),the positive rate of CD56 was 88%(n=44),and the positive rate of P40 was 8%(n=4).The results of Cox hazard model analysis suggested that TTF-1 was an independent factor affecting the prognosis of patients with small cell lung cancer(P<0.05).There was no correlation between the positive expression of CD56 and P40 and the prognosis of patients with small cell lung cancer(P>0.05).In addition to TTF-1,the independent factors affecting the prognosis of small cell lung cancer patients included smoking index,ECOG score,and whether patients had superior vena cava syndrome(all P<0.05).Conclusion The independent factors affecting the prognosis of small cell lung cancer patients were TTF-1,smoking index,ECOG score and whether the patients had superior vena cava syndrome.Examination of the expression of TTF-1,CD56 and P40 favors the clinical diagnosis and differentiation of small cell lung cancer.
