摘要
目的研究滋阴疏肝汤联合八髎穴隔姜灸治疗肾虚肝郁型卵巢功能低下的临床疗效。方法将2017年1月至2018年9月我院妇科接诊肾虚肝郁型卵巢功能低下患者94例纳入研究,依随机数表法分对照组、研究组,各47例。对照组采用滋阴疏肝汤治疗,研究组患者选择滋阴疏肝汤联合八髎穴隔姜灸治疗。对比2组患者治疗前后中医症候积分、性激素水平、卵巢功能及妊娠率。结果治疗后2组中医症候积分降低,研究组更低,差异有统计学意义(P<0.05);治疗后2组患者的血清雌二醇(E2)、抗缪勒管激素(AMH)指标升高,研究组更高,差异有统计学意义(P<0.05);治疗后2组血清卵泡刺激素(FSH)、黄体生成素(LH)指标降低,研究组更低,差异有统计学意义(P<0.05);治疗后2组卵巢功能改善,研究组改善效果更佳,差异有统计学意义(P<0.05);研究组治疗后1年的妊娠率66%(31/47)高于对照组43%(20/47),差异有统计学意义(P<0.05)。结论滋阴疏肝汤联合八髎穴隔姜灸治疗肾虚肝郁型卵巢功能低下的临床疗效确切,可有效缓解症状体征,调节性激素指标表达,进而改善患者的卵巢功能,提高患者妊娠率,值得临床推广。
Objective To determine the clinical effect of Ziyin Shugan decoction combined with ginger-partitioned moxibustion at eight-hole acupoints in the treatment of kidney deficiency and liver depression type ovarian failure.Methods Ninety-four patients with kidney deficiency and liver depression type ovarian failure admitted to the Department of Gynecology in our hospital between January 2017 and September 2018 were included in the study.All patients were randomly divided into the control group and the study group(n=47 each).The control group was treated with Ziyin Shugan decoction,and the study group was treated with Ziyin Shugan decoction and ginger-partitioned moxibustion at eight-hole acupoints.The TCM syndrome scores,sex hormone level,ovarian function and pregnancy rate at baseline and after the treatment were compared between the two groups.Results After the treatment,the TCM syndrome scores decreased in the two groups,and the scores in the study group were lower than those in the control group,with statistically significant differences(all P<0.05).After the treatment,the serum levels of E2 and AMH increased in the two groups,and the levels in the study group were higher than those in the control group,with statistically significant differences(both P<0.05).The serum levels of FSH and LH in the two groups decreased after the treatment,and the levels in the study group were lower than those in the control group,with statistically significant differences(both P<0.05).The ovarian function in the two groups was improved after the treatment,and the improvement effect in the study group was better than that in the control group,with statistically significant difference(P<0.05).The pregnancy rate at 1 year after the treatment in the study group was higher than that in the control group[66%(31/47)vs 43%(20/47)],with statistically significant difference(P<0.05).Conclusion Ziyin Shugan decoction combined with ginger-partitioned moxibustion at eight-hole acupoints is effective in the treatment of kidney deficiency and liver depression type ovarian failure.It may effectively relieve the symptoms and signs,regulate sex hormone levels,thereby improving ovarian function and pregnancy rate,which justifies widespread use in the clinical settings.
