18F-FDG PET/CT非小细胞肺癌原发灶代谢参数和纵隔淋巴结SUVmax预测纵隔淋巴结转移的价值 被引量:16

摘要 目的探究^18F-FDG PET/CT原发灶代谢参数和淋巴结最大标准摄取值(N_SUVmax)预测非小细胞肺癌(NSCLC)纵隔淋巴结转移的价值。方法选取接受^18F-FDG PET/CT检查并符合入选标准非小细胞肺癌患者56例,对原发灶代谢异常区勾画相应感兴趣区,获得原发灶最大标准摄取值(T_SUVmax)、糖酵解总量(T_TLG)、代谢体积(T_MTV)值,并测量淋巴结最大标准摄取值(N_SUVmax),根据病理结果分为淋巴结转移组(n=26)以及无淋巴结转移组(n=30),比较两组原发灶代谢参数和淋巴结最大标准摄取值的差异以及分析其与非小细胞肺癌纵隔淋巴结转移的关系。结果56例NSCLC患者原发灶大小、T_SUV max、T_TLG、T_MTV、N_SUVmax差异均有统计学意义(P<0.05)。T_SUVmax的ROC曲线下面积为0.933(P<0.001),当T_SUVmax值为11.29时,是预测淋巴结转移的界值。T_TLG的ROC曲线下面积为0.939(P<0.001),当T_TLG值为40.58 g时,是预测淋巴结转移的界值。T_MTV的ROC曲线下面积为0.669(P<0.05),当T_MTV值为10.37 cm^3时,是预测淋巴结转移的界值。N_SUVmax的ROC曲线下面积为0.847(P<0.001),界值为5.15。结论非小细胞肺癌原发灶大小、原发灶最大标准摄取值(T_SUV max)、糖酵解总量(T_TLG)、代谢体积(T_MTV)、淋巴结最大标准摄取值(N_SUVmax)与纵隔淋巴结转移有关,原发灶大小、原发灶代谢参数及N_SUVmax对诊断非小细胞肺癌患者纵隔淋巴结转移具有预测价值。 Objective To explore the correlation between the metabolic parameters of ^18F-FDG PET/CT in primary tumor and maximum standardized uptake value of regional lymph node(NSUVmax)and mediastinum lymph node metastasis in non-small cell lung cancer.Methods A total of 56 patients with non-small cell lung cancer who underwent ^18F-FDG PET/CT examination were enrolled.The size of the primary lesion was measured and the corresponding region of interest was delineated from the original metabolic abnormality zone.Maximum standardized uptake value of primary tumor(TSUVmax),total lesion glycolysis of primary tumor(TTLG),metabolic tumor volume of primary tumor(TMTV)and maximum standardized uptake value of regional lymph node(NSUVmax)were obtained.The patients were divided into lymph node metastasis group(n=26)and no lymph node metastasis group(n=30).The differences of primary tumor parameters and NSUVmax and their relationship with mediastinum lymph node metastasis of non-small cell lung cancer were compared.Results There were significant differences in primary lesion size,TSUV max,TTLG,TMTV,and NSUVmax in 56 patients with NSCLC(P<0.05).The area under the ROC curve of the primary standard uptake value of the primary tumor was0.933(P<0.001).When the maximum standardized uptake value of primary tumor was 11.29,it was the boundary value for predicting lymph node metastasis.The area under the ROC curve of total lesion glycolysis of primary tumor was 0.939(P<0.001).When the TTLG value was 40.58 g,it was the predicted margin of lymph node metastasis.The area under the ROC curve of the primary tumor MTV was 0.669(P<0.05).When the MTV value of the primary tumor was 10.37 cm^3,it was the boundary value for predicting lymph node metastasis.The area under the ROC curve for NSUVmax was 0.847(P<0.001)with a cut off of 5.15.Conclusions The size of the primary tumor of non-small cell lung cancer,the maximum standard uptake value of the primary tumor(TSUV max),the total amount of glycolysis(TTLG),the metabolic volume(TMTV),and the maximum standard uptake value of lymph nodes(NSUVmax)were all related with mediastinum lymph node metastasis.The size of the primary tumor,the metabolic parameters of the primary tumor and NSUVmax had predictive value for the diagnosis of mediastinum lymph node metastasis in patients with non-small cell lung cancer.
