摘要
目的:探究母乳联合碳酸氢钠口腔护理预防新生儿呼吸机相关性肺炎的作用。方法:选取100例2018年1-12月笔者所在医院新生儿重症监护病房(NICU)应用呼吸机辅助通气治疗的患儿为研究对象,最终纳入96例,随机分成A组(n=49)和B组(n=47)。B组采用使用0.9%氯化钠液进行口腔护理,A组采用母乳联合碳酸氢钠进行口腔护理。观察记录两组患儿口腔清洁度评分、致病微生物培养阳性率、VAP发生率、ICU住院时间及机械通气时间。结果:A组口腔清洁度评分高于B组,差异有统计学意义(P<0.05);A组患儿致病微生物培养阳性率、VAP发生率低于B组,ICU住院时间及机械通气时间均短于B组,差异均有统计学意义(P<0.05)。结论:ICU患儿采用母乳联合碳酸氢钠口腔护理的护理模式,有助于更好地清洁口腔,降低VAP发生率,帮助患儿早期脱机拔管,减少ICU住院时间,降低并发症的发生率,值得推广使用。
Objective:To explore the effect of breast milk combined with Sodium Bicarbonate oral nursing on preventing ventilator-associated pneumonia in newborns.Method:Selected 100 children who were treated with ventilator assisted ventilation of neonatal intensive care unit(NICU)in our hospital from January to December 2018.The 96 children finally included were randomly divided into group A(n=49)and group B(n=47)cases.Group B used 0.9%Sodium Chloride Solution for oral cavity nursing,and group A used breast milk combined with Sodium Bicarbonate for oral nursing.Oral cleanliness score,positive rate of pathogenic microorganism culture,incidence of VAP,hospitalization time of ICU,time of mechancial ventilation were observed and recorded.Result:The oral cleanliness score of group A was higher than that of group B,the difference was statistically significant(P<0.05).The positive rate of pathogenic microorganism culture and the incidence of VAP of group A were lower than those of group B,and ICU hospitalization time and time of mechancial ventilation in group A were shorter than those in group B,the differences were statistically significant(P<0.05).Conclusion:The nursing mode of breast milk combined with Sodium Bicarbonate oral care for ICU children is helpful to better clean the mouth.It can reduce the incidence of VAP,help children to extubate early,reduce the length of stay in ICU,and reduce the incidence of complications.It is worth popularizing.
