摘要
目的:观察中医综合疗法防治老年髋部骨折术后下肢深静脉血栓(deep venous thrombosis,DVT)的效果。方法:选取2016年5月-2017年5月在笔者所在医院行手术治疗的老年髋部骨折患者50例,随机分为对照组和观察组,每组25例。对照组给予肝素注射液治疗,观察组在对照组基础上给予中药汤剂、中药包熨烫、中药离子导入等中医综合疗法治疗。治疗后,比较两组治疗总有效率、治疗前后患肢肿胀程度、下肢深静脉血栓(DVT)及凝血酶原时间(PT)异常发生率、治疗期间不良反应发生情况。结果:治疗后,观察组治疗总有效率高于对照组,下肢DVT及PT异常发生率均低于对照组,差异均有统计学意义(P<0.05);治疗3、7 d后观察组患肢小腿周径均短于对照组,差异均有统计学意义(P<0.05);两组不良反应发生率比较差异无统计学意义(P>0.05)。结论:中医综合疗法防治老年髋部骨折术后下肢深静脉血栓效果好,能够改善患者患肢肿胀程度及凝血功能,降低下肢静脉血栓发生率。
Objective:To observe curative effect of traditional Chinese medicine(TCM)comprehensive therapy on controlling deep venous thrombosis(DVT)of lower limbs in the elderly after hip fracture surgery.Method:A total of 50 elderly patients with hip fracture who underwent surgery in the hospital from May 2016 to May 2017 were enrolled.They were randomly divided into control group and observation group,25 cases in each group.The control group was given heparin injection.On this basis,observation group was given TCM comprehensive therapy such as TCM decoction,TCM bag ironing and TCM iontophoresis.The incidence of limb swelling,the incidence of deep venous thrombosis and the incidence of prothrombin time(PT),the incidence of adverse reactions during treatment were compared between the two groups.Result:After treatment,the total effective rate of treatment in the observation group was higher than that in the control group,and the abnormal incidence of DVT and PT in the lower extremity was lower than that in the control group,the differences were statistically significant(P<0.05).After 3,7 days of treatment,the circumference of leg in the observation group were shorter than those in the control group,the differences were statistically significant(P<0.05).There was no significant difference in occurrence of adverse reactions between the two groups(P>0.05).Conclusion:The comprehensive treatment of traditional Chinese medicine has good effect on the deep venous thrombosis of the lower limbs after the treatment of the old hip fracture,and can improve the swelling degree and the blood coagulation function of the affected limb of the patient,and reduce the incidence of the venous thrombosis of the lower limb.
