摘要
目的分析头颈部鼻型结外NK/T细胞淋巴瘤(extranodal NK/T-cell lymphoma,nasal type,ENKTCL)误诊病例,提高认识以避免误诊。方法回顾分析8例存在多次误诊误治头颈部ENKTCL的诊治经过。同时在数据库上查找同期关于头颈部ENKTCL误诊病例报道文献一并进行分析。结果本研究8例病例误诊次数 3~4次,误诊疾病有鼻窦炎、鼻出血、上呼吸道感染等。查到符合条件的文献共21篇,误诊疾病有鼻窦炎、面部蜂窝织炎、鼻炎等。导致误诊的原因有早期临床表现缺乏特异性、影像学表现缺乏明显特征和病理学特征的复杂性。结论 ENKTCL早期容易发生误诊,加强对其临床、影像和病理特征的认识,有助于及早识别ENKTCL,及时准确取病理才能尽可能避免误诊误治。
OBJECTIVE The clinical data of misdiagnosed cases of extranodal nasal NK/T cell lymphoma (ENKTCL) in head and neck were analyzed in order to improve the understanding and avoid misdiagnosis. METHODS The process of diagnosis and treatment of 8 patients with ENKTCL in head and neck, whom were multiple misdiagnosed and mistreated,was analyzed. At the same time, the literatures about the misdiagnosed cases of ENKTCL in head and neck, which were from document database, were also analyzed. RESULTS In this study, 8 cases were misdiagnosed 3 to 4 times, and misdiagnosed diseases included chronic sinusitis, epistaxis, upper respiratory tract infection, etc. A total of 21 qualified literatures were found, with 23 cases in all, and misdiagnosed diseases included chronic sinusitis, acute sinusitis, facial cellulitis, etc. The causes of misdiagnosis contained the lack of specific clinical manifestation and imaging features in the early stage of ENKTCL, and the complexity in pathological features. CONCLUSION Misdiagnosis is easy to occur in the early stage of ENKTCL. So it is an important measure to avoid misdiagnosis by strengthening the understanding of the clinical, imaging and pathological features of ENKTCL, and early identifying of ENKTCL.
