摘要
目的:观察清肺敛阳纳气方治疗慢性阻塞性肺疾病(COPD)合并肺心病的临床疗效。方法:采用前瞻性研究,将82例COPD合并肺心病患者按随机数表法分为治疗组和对照组,均给予常规西医治疗,治疗组加用清肺敛阳纳气方,治疗14d。观察并比较治疗前后咳嗽、喘息、咯痰、下肢浮肿等主要症状评分以及肺功能指标、感染指标、心功能指标,比较临床疗效。结果:①治疗后,治疗组与对照组症状均有不同程度改善,P<0.05;与对照组相比,清肺敛阳纳气方能显著改善咳痰、喘息症状,P<0.01;在咳嗽、口唇紫绀、双下肢浮肿等症状改善方面也优于常规西医治疗的对照组,P<0.05。②治疗后,两组在肺功能、心功能指标及感染指标方面均有不同程度的改善,P<0.01;而与对照组相比,清肺敛阳纳气方在改善心肺功能、降低炎症指标方面优于常规西医治疗,P<0.01。③治疗组总有效率为97.56%,明显优于对照组的90.24%,差异有统计学意义(P<0.05);治疗组显效率明显高于对照组,差异有统计学意义(P<0.01)。结论:清肺敛阳纳气方能有效缓解慢性阻塞性肺疾病合并肺心病症状,改善肺功能,降低感染指标,改善心功能水平,效果优于常规治疗。
Objective:To observe the clinical effect of Qingfeilianyang naqi decoction in the treatment of chronic obstructive pulmonary disease combined(COPD)with pulmonary heart disease.Methods:Using prospective study,82 patients with COPD were divided into treatment group and control group according to random number table method.Both treatment group were given routine treatment.The treatment group added traditional Chinese medicine Qingfeilianyang naqi decoction,Take it twice in the morning and evening for 14 days.To observe and compare two groups of patients before and after treatment cough,wheezing,clotting sputum,lower extremity edema and other major symptom scores as well as lung function index,infection index and cardiac function index changes,compare two groups of patients clinical efficacy.Results:①After the treatment,the symptoms of each group in the treatment group and the control group improved to different degrees.Among them,compared with the control group,the Qingfeilianyang naqi decoction can significantly improve the patient's cough and breathing symptoms(P<0.05);In cough,lip cyanosis,lower extremity edema and other symptoms of the improvement is also better than conventional Western medicine treatment control group.P<0.05.②After treatment,the two groups of patients had different degrees of improvement in lung function,cardiac function index and infection index(P<0.01),compared with the control group,The Qingfeilianyang naqi decoction is superior to conventional Western medicine in improving cardiopulmonary function and reducing inflammation index.③The total efficiency of treatment group was 97.56%,which was superior to 90.24%of the control group.The difference was statistically significant(P<0.05).The apparent efficiency of treatment group was significantly higher than that of control group.The difference was statistically significant(P<0.01).Conclusion:Qingfeilianyang naqi decoction can effectively relieve the main disease of patients with chronic obstructive pulmonary disease combined with pulmonary heart disease,improve the pulmonary function index,reduce the infection index and improve the level of cardiac function,and its effect is better than conventional treatment.
