摘要
目的评价危重症患者肠内营养并发胃残余处理的相关指南,比较各指南的异同,为临床循证决策及指南制定提供参考。方法系统检索JBI、国际指南协助网、美国国家指南网等网站及数据库中的危重症患者肠内营养指南,检索时限为自2009年1月1日至2018年12月31日。采用临床指南研究和质量评价工具(the appraisal of guidelines for researchevaluation instrument,AGREEⅡ)对符合纳入、排除标准的指南进行质量评价。结果共纳入5篇指南,4篇来自国外,1篇来自国内;5篇指南在6个领域的AGREEⅡ评价平均得分分别为:范围和目的83.34%,参与人员54.02%,严谨性69.58%,清晰性74.82%,应用性46.39%和独立性61.67%。指南评价结果中,2篇指南被评为A级,3篇指南被评为B级。结论指南推荐内容中胃残余判断标准国内外有差别,胃残余处理意见大致相同。
Objective To evaluate the relevant guidelines for gastric residual management in critical patients with enteral nutrition,and to compare the similarities and differences of the guidelines,so as to provide the reference for clinical evidence-based decision-making and guideline formulation.Methods Clinical research and evaluation tool AGREEⅡwas used to evaluate the quality of related evidence-based guidelines obtained from the website search and database search,and analyze the similarities and differences of recommended contents in each guideline.Results A total of five guidelines were included,four from abroad and one from China.The average scores of the five guidelines in the six areas of AGREEⅡevaluation were 83.34%for scope and purpose,54.02%for participants,69.58%for rigor,74.82%for clarity,46.39%for applicability and 61.67%for independence.In the evaluation results of the guidelines,two guidelines were rated as grade A and three guidelines were rated as grade B.Conclusions There are differences between domestic and foreign standards for the determination of gastric residual in the recommended contents of the guidelines,and opinions on the treatment of gastric residual are roughly the same.
