摘要
目的:分析老年髋部骨折翻身循证护理的应用效果。方法:选取连州市人民医院骨科髋部骨折后至手术前患者180例,随机分为试验组和对照组,各90例。对照组采取传统护理和体位指导,即患侧位-平卧位-健侧位交替翻身。试验组采取翻身循证护理,主要针对以往体位存在的问题进行改进,实行向患侧30°翻身法:每2~3 h为患者实施患侧30°-平卧位交替翻身,避免向健侧位翻身;患肢臀部、粗隆间等易受压部位贴安普贴薄膜减压。比较两组翻身时疼痛程度、主观舒适度评分及翻身护理时间。结果:试验组翻身时疼痛程度、主观舒适度评分、翻身护理时间均优于对照组(P<0.05)。结论:翻身循证护理在老年髋部骨折后至手术前护理中的应用效果确切,可更好提高患者的主观舒适度,减轻翻身疼痛感,缩短翻身护理时间。
Objective:To analyze the application effect of turning-over evidence-based nursing in elderly hip fracture.Method:A total of 180 patients with hip fracture before operation from orthopedic department in Lianzhou People’s Hospital were selected.Patients were randomly divided into the experimental group and the control group,90 cases in each group.The control group received traditional nursing and posture guidance,that is,the affected side position-supine position-healthy side position alternately turn over.In the experimental group,turning-over evidence-based nursing was adopted to improve the existing problems in the previous position,and the 30°turning to the affected side was implemented.Every 2~3 hours,patients were alternately turned to the affected side in 30°-supine position to avoid turning to the healthy side.The hip of the affected limb,intertrochanteric and other vulnerable parts using Algoplaque Film to decompress.The degree of pain at turning over,the scores of subjective comfort,and the nursing time of turning over were compared between the two groups.Result:The degree of pain at turning over,the scores of subjective comfort and the nursing time of turning over of the experimental group were better than those of the control group(P<0.05).Conclusion:The application of turning-over evidence-based nursing in the elderly patients with hip fracture before operation is effective,which can improve the subjective comfort of patients,reduce the pain at turning over,and shorten the nursing time of turning over.
