摘要
目的:探讨益气养心汤联合比索洛尔对慢性心力衰竭(CHF)患者血管内皮功能及心功能的影响。方法:选取笔者所在医院2016年7月-2018年7月收治的108例慢性心力衰竭患者,按照随机数字表法将其分为对照组和观察组,每组54例。对照组患者采取富马酸比索洛尔片治疗,观察组在对照组基础上给予益气养心汤治疗。比较两组临床疗效、治疗前后中医证候及Lee氏心衰积分、左室射血分数(LVEF)及每搏量(SV)、一氧化氮(NO)和内皮素(ET-1)。结果:观察组治疗总有效率高于对照组(96.30%vs 83.33%),差异有统计学意义(χ^2=4.960,P=0.026)。两组治疗后中医证候及Lee氏心衰积分均较治疗前降低,且观察组均低于对照组(P<0.05)。两组治疗前LVEF和SV水平比较差异无统计学意义(P>0.05);观察组治疗后LVEF水平高于对照组,SV水平高于对照组(P<0.05)。两组治疗前NO和ET-1水平比较差异均无统计学意义(P>0.05);观察组治疗后NO水平高于对照组,ET-1水平低于对照组(P<0.05)。结论:益气养心汤联合比索洛尔对慢性心力衰竭患者疗效较好,可有效提高其血管内皮功能及心功能,改善其中医症状。
Objective:To study the effects of Yiqi Yangxin Decoction and Bisoprolol on the vascular endothelial function and cardiac function in patients with chronic heart failure(CHF).Method:A total of 108 patients with chronic heart failure admitted in our hospital from July 2016 to July 2018 were selected and divided into the control group and the observation group according to the random number table method,with 54 cases in each group.The control group was treated with bisopropiolol fumarate tablets.The observation group was treated with Yiqi Yangxin Decoction on the basis of the control group.The Clinical efficacy,TCM syndrome and Lee’s heart failurescores before and after treatment,left ventricular ejection fraction(LVEF)and stroke volume(SV)and nitric oxide(NO)and endothelin(ET-1)were compared between the two groups.Result:The totaleffective rate of treatment in the observation group was higher than that in the control group(96.30%vs 83.33%),the difference was statistically significant(χ^2=4.960,P=0.026).After treatment,theTCM syndrome and Lee’s heart failure scores of both groups were lower than before treatment,and the observation group was lower than the control group(P<0.05).There were no significant differences in LVEF and SV levels between the two groups before treatment(P>0.05).After treatment,the LVEF level in the observation group was higher than that in the control group,and SV level was higher than that in the control group(P<0.05).There were NO significant differences in the levels of NO and ET-1 between the two groups before treatment(P>0.05).After treatment,NO level in the observation group was higher than that in the control group,and ET-1level was lower than that in the control group(P<0.05).Conclusion:Yiqi Yangxin Decoction combined with Bisoprolol has a good effect on patients with chronic heart failure,and can effectively improve their vascular endothelial function and cardiac function,and improve the symptoms of TCM.
