摘要
脾胃强弱在《伤寒论》六经病的发生、传变中起重要作用。脾胃之气不足,营卫之气不能外达,卫外功能下降,外邪侵袭人体而发太阳病;胃为阳土,若阳明热盛,热盛伤津,胃气不降,腑气不通,燥实互结,则发为阳明腑证;少阳病的发病前提是脾胃虚弱,致正气不足,邪气侵袭而发病;阳气不足,则阴寒邪气易直中太阴,出现阳虚寒盛之证;肾阳不足,邪气直中少阴,发为少阴病;中气不运,胃气易逆,脾阳易陷,则阴阳不相顺接,发为厥阴病。《伤寒论》中大量使用生姜、甘草、大枣顾护脾胃,在运用峻下祛邪治法中,亦不忘顾护脾胃,在煎煮和服用药物的过程中也时刻注重脾胃,方后的药后宜忌与注意事项,也体现了张仲景固护脾胃的学术思想。辨析脾胃之气的盛衰对掌握疾病发展传变具有重要的指导意义。
The strength of spleen and stomach plays an important role in the occurrence and transmission of six meridian diseases in Treatise on Cold Damage Diseases.If qi of spleen and stomach is insufficient,the qi of ying and wei can′t reach outside,the defense function of weiqi decreases,the external pathogen invades the human body and causes Taiyang disease.The stomach is yang soil.If Yangming is excessively hot,and stomach qi does not fall,the viscera qi will be blocked,and the dryness and the solid will be combined,and the syndrome of Yangming will occur.The premise of Shaoyang disease is the weakness of spleen and stomach leading to the deficiency of healthy qi and the invasion of pathogenic qi.If yang qi is insufficient,then yin cold and pathogenic qi will easily enter Taiyin meridian,and the syndrome of yang deficiency and cold excess will appear.The kidney yang is insufficient,the pathogenic qi enter Shaoyin meridian,and Shaoyin disease occurs.If the middle qi is not moving,the stomach qi is reverse,the spleen yang will be easy to sink,then the yin and yang are not smoothly connected,resulting in Jueyin disease.In Treatise on Cold Damage Diseases,Shengjiang(Rhizoma Zingiberis Recens),Gancao(Radix et Rhizoma Glycyrrhizae) and Dazao(Fructus Jujubae) are widely used to protect the spleen and stomach.In the treatment of removing pathogenic factors by using medicine for drastic purgation,protecting the spleen and stomach is still not neglected.In the process of decocting and taking medicine,the spleen and stomach are always paid attention to.Zhang Zhong-jing′s academic thought of protecting the spleen and stomach is also reflected in appropriate taboos and precautions after taking medicine It is of great significance to analyze the rise and fall of qi in spleen and stomach.
