摘要
阵发性房颤(paroxysmal atrial fibrillation,PAF)可归属于中医"心悸""怔仲"等范畴,其发病部位在心之络脉,络脉生风动风导致心神惊悸不安是诱发PAF的病理学基础。因此,可将"络风内动"视为PAF的核心病机。熄风宁络法治疗PAF主要体现在风药的配伍及具体运用上:络虚失荣,生风动风者,可扶正与熄风兼顾;热盛动风者,当熄风与化痰、理血相伍;外风引动内风者,当除外以安内,可重用熄风药为君药、臣药,令玄府开泻有度,气机条达,则悸动自安;病情迁延不愈,风邪内伏者,取虫类风药克之,如此则痼疾清,沉疴安,悸动平。然风性属阳,入络化燥,久之必耗气、伤津、动血,故各期变化,需平调气、血、津液。
Paroxysmal atrial fibrillation(PAF) can be attributed to " heart palpitations" or " being terified" in traditional Chinese medicine,and the location of the disease is in the heart collaterals. The collateral Wind causing convulsions is the pathological basis of PAF. Therefore,collateral Wind can be regarded as the core pathogenesis of PAF. The treatment of PAF with Xifeng Ningluo method is mainly reflected in the compatibility and specific application of the Wind medicines:when there is deficiency in collaterals,the Wind would exist,the treatment should be tonifying righteous Qi and the stopping Wind;in case of Hotness induced Wind,stopping Wind,methods of regulating Blood and removing Phlegm should be used. If it is the outside Wind inducing the internal Wind,the outside Wind should be dispelled to stop the inside Wind,and the Wind-extinguishing drugs can be used as Jun drug or Chen drug,so that the Xuanfu can be opened to a proper degree and the Qi movement would be normal. When the disease lasts long with Wind Evil being insider,the insect drugs can be used to stop Wind. In this way,the disease would be cured. However,the Wind Evil is Yang Evil,and it consumes Qi,Fluid and Blood. Therefore,it is necessary to adjust the Qi,Blood and body Fluid conditions during the whole process of the treatment.
