陆军新兵军事训练对骨密度及骨代谢指标的影响 被引量:2

Effects of military training on the bone density and bone metabolism index of the army recruits

摘要 目的分析陆军新兵军事训练对骨密度(BMD)及血清骨代谢指标的影响。方法于2009年9月招募的某部陆军汉族男性新兵275名,年龄17~23(19.3±1.5)岁,按照原总参谋部批准的2009版《军事训练与考核大纲》进行3个月的新兵军事训练。训练前后分别测量身高、体重,计算体重指数(BMI);利用双能X线仪(DXA)检测腰椎1–4(L1–4)、左侧股骨颈(FN)及全髋(TH)3个部位的BMD;化学发光法检测血清骨形成标志物骨碱性磷酸酶(BALP)水平,ELISA法检测血清骨吸收标志物抗酒石酸酸性磷酸酶5b(TRAP5b)水平。结果与训练前比较,新兵军事训练后身高有所增高,体重及BMI降低(P<0.05);L1–4、FN和TH 3个部位的BMD均升高(P<0.05);血清BALP水平明显升高,TRACP5b水平明显降低(P<0.05)。相关分析结果显示,年龄与训练前后L1–4 BMD变化率(r=–0.160,P=0.009)及TH BMD变化率均呈负相关(r=–0.235,P<0.001),与FN BMD变化率无明显相关性(r=–0.111,P=0.074);年龄与TRACP5b变化率呈正相关(r=–0.264,P=0.041),与BALP变化率无明显相关性(r=–0.060,P=0.636)。L1–4 BMD、TH BMD变化率与BALP变化率均呈正相关(r分别为0.291、0.272,P分别为0.019、0.028),与TRACP5b变化率均无明显相关性(r分别为0.045、–0.102,P分别为0.724、0.417);FN BMD变化率与BALP及TRACP5b变化率均无明显相关性(r分别为0.051、–0.137,P分别为0.685、0.277)。结论陆军新兵军事训练能明显促进骨形成,可将BMD和血清骨代谢指标作为新兵骨骼健康的评价指标。 Objective To evaluate the effects of a 3-month military training on the bone mineral density(BMD)and serum bone metabolism indexes of the army recruits.Methods A total of 275 male Han recruits,aged 17-23(19.3±1.5)years,were involved in present study,and underwent a 3-month military training according to the 2009 edition of the"Outline of Military Training and Assessment".All the parameters were evaluated before and after the military training including height,weight,and body mass index(BMI).Dual energy X-ray absorptiometry(DXA)was employed to check the BMD of lumbar vertebra 1-4(L1-4),Left femoral neck(FN)and total hip(TH);Chemiluminescence method was used to detect the bone specific alkaline phosphatase(BALP)level in serum bone formation marker;and ELISA was performed to detect the anti-tartrate 5b(TRAP5b)levels in serum bone resorption markers.Results There's significant change of height,weight and BMI before and after training(P<0.05).Comparing with the baseline,3-month military training significantly increased the BMD of L1-4,FN and TH and serum BALP level,while decreased the serum TRACP5b level(P<0.05).Correlation analysis showed a significantly negative correlation between age and the BMD change ratio of L1-4(r=–0.160,P=0.009)and of TH(r=–0.235,P<0.001);A positive correlation existed between age and the change ratio of TRACP5b(r=0.264,P=0.041),but no obvious correlation between age and the change ratio of FN BMD(r=–0.111,P=0.074),or of the serum BALP concentration(r=–0.060,P=0.636).The change ratio of serum BALP level was positively correlated with that of L1-4 BMD(r=0.291,P=0.019)and TH BMD(r=0.272,P=0.028),but no significant correlation existed between the change ratio of TRACP5b and that of L1-4 BMD(r=0.045,P=0.724)and TH BMD(r=–0.102,P=0.417).There's no significant correlation between the change ratio of FN BMD and that of serum BALP(r=0.051,P=0.685)and TRACP5b(r=–0.137,P=0.277).Conclusions Military training of army recruits may obviously promote the bone formation.BMD and serum bone metabolism indexes may be used to evaluate the bone health status of recruit.
