摘要
目的:回顾性分析雷珠单抗注射联合玻璃体切除和硅油内注射曲安奈德(TA)治疗增殖性糖尿病视网膜病变(PDR)的疗效。方法:回顾性分析53例(53眼)在玻璃体切除术前注射雷珠单抗,玻璃体切除后行硅油填充的PDR患者资料,其中25眼硅油内注射了TA 2 mg。测定术前、术后1 w、术后1 m和术后3 m的术眼眼压(IOP)和黄斑视网膜厚度(CRT)。结果:两组术前IOP差异无统计学意义(P>0.05)。术后1 w,TA组IOP明显低于非TA组(P<0.05)。术后1 m和术后3 m,两组IOP差异无统计学意义(均P>0.05)。术后1 m与术后1 w相比,两组的CRT均明显降低(均P<0.05);术后3 m与术后1 m相比,非TA组CRT变化无统计学差异(P>0.05),TA组CRT明显降低(P<0.05)。结论:玻璃体切除术前注射雷珠单抗联合术中硅油内注射TA 2 mg治疗PDR,在术后3个月内可取得CRT持续减低的效果,并且IOP未见明显升高。
Objective:To observe the clinical effect of intravitreal injection with ranibizumab combined with vitrectomy and triamcinolone acetonide(TA)injected in silicone oil on proliferative diabetic retinopathy(PDR).Methods:A total of 53 eyes from 53 patients with PDR were chosen for retrospective analysis.All of these cases were injected with ranibizumab before vitrectomy and silicone tamponade.There were 25 eyes given TA at a dose of 2 mg in silicone oil for addition.Intraocular pressure(IOP)and central retinal thickness(CRT)were measured before vitrectomy,and one week,one month and three months postoperative.Results:There was no significant difference of IOP between two groups before vitrectomy(P>0.05).The IOP was lower in TA group when compared with non-TA group at one week postoperative(P<0.05).However,there was no significant difference of IOP between the two groups both at one month and[JP2]three months after vitrectomy,respectively(both P>0.05).Compared with one week after the operation,[JP]CRT in these two group were both significantly decreased at one month postoperative(both P<0.05).CRT at three months after operation had no statistical changes in the non-TA group(P>0.05),while performed an obvious decrease in the TA group(P<0.05),when compared with one month postoperative.Conclusion:A dose of 2 mg TA injected in silicone oil for addition in PDR patients who were intravitreal injected with ranibizumab before vitrectomy could reduce CRT continuously,with no significance increase of IOP in 3 months follow-up.
