摘要
目的探讨老年维持性血液透析患者医院感染现状及其危险因素。方法选取我院2017年3月至2019年3月收治的老年维持性血液透析患者124例,回顾性分析患者临床资料,统计发生医院感染的例数,并借助多因素logistic回归分析筛选老年维持性血液透析患者医院感染危险因素。结果所选研究对象中,发生医院感染41例(占33.06%);多因素logistic回归分析确定老年维持性血液透析患者医院感染独立危险因素包括高龄(≥65岁)、置管留置时间(≥2个月)、合并糖尿病、透析时间(>10个月)、使用抗生素、自护水平低(<90分)。结论我院老年维持性血液透析患者医院感染发生率处于中等水平,其危险因素较多,临床需采取针对性措施,加大医院感染防控力度,确保患者就医安全。
Objective To investigate the status and risk factors of nosocomial infection in elderly patients with maintenance hemodialysis.Methods To select 124 cases of elderly patients with maintenance hemodialysis who admitted to our hospital from March 2017 to March 2019.The clinical data of the patients were retrospectively analyzed,the number of cases of nosocomial infections was counted,and the nosocomial infection and risk factors in elderly patients with maintenance hemodialysis was screened by multivariate logistic regression analysis to analyze the risk factors.Results Among the selected subjects,41 cases(33.06%)had nosocomial infection.Multivariate logistic regression analysis showed that the independent risk factors of nosocomial infections in elderly patients with maintenance hemodialysis included advanced age(≥65 years old),indwelling time(≥2 months),combined diabetes,dialysis time(>10 months),use of antibiotic,and low self-care level(<90 points).Conclusion The incidence of nosocomial infections in elderly patients with maintenance hemodialysis in our hospital was at a medium level and there were many risk factors.It was necessary to take targeted measures clinically to increase the prevention and control of nosocomial infections to ensure the safety of patients seeking medical treatment.
