摘要
目的探讨结直肠癌造口患者造口周围性皮炎发生的相关影响因素。方法选取2018年1月至2019年5月我院收治的结直肠癌造口患者186例为研究对象,回顾性分析社会人口学资料和临床病历资料,采用多因素logistic回归分析确定结直肠癌造口患者造口周围性皮炎发生的独立危险因素。结果186例患者中发生造口周围性皮炎58例(31.18%),其中粪水性皮炎23例,真菌感染16例,毛囊炎11例,外界刺激损伤8例。多因素logistic回归分析显示,辅助化疗、辅助放疗、造口产品及造口类型是结直肠癌造口患者发生造口周围性皮炎的独立危险因素(P<0.05),造口护理知识是结直肠癌造口患者发生造口周围性皮炎的保护因素(P<0.05)。结论导致结直肠癌造口患者造口周围性皮炎发生的影响因素较多,为此应加强造口患者的自我管理能力,提升其造口相关护理知识认知水平,制定规范化的预防机制,减少结直肠癌造口患者造口周围性皮炎发生率,减轻身心痛苦,提高术后生活质量。
Objective To investigate the related influencing factors of peripheral dermatitis due to neostomy in patients with colorectal cancer.Methods To select 186 cases of patients with colorectal cancer undergoing neostomy who treated in our hospital from January 2018 to May 2019 as the study objects.The sociodemographic data and clinical medical records were retrospectively analyzed.Multivariate logistic regression analysis was used to determine the independent risk factor for peripheral dermatitis in patients with colorectal cancer undergoing neostomy.Results There were 58 cases(31.18%)of peripheral dermatitis that occurred in 186 cases of patients,including 23 cases of faecal dermatitis,16 cases of fungal infection,11 cases of folliculitis,and 8 cases of external stimulus injury.Multivariate logistic regression analysis showed that the adjuvant chemotherapy,adjuvant radiotherapy,products for neostomy and neostomy types were the independent risk factors for peripheral dermatitis in patients with colorectal cancer undergoing neostomy(P<0.05).and the nursing knowledge of neostomy was the protective factor for peripheral dermatitis in patients with colorectal cancer undergoing neostomy(P<0.05).Conclusion There were many influential factors that led to the occurrence of peripheral dermatitis in patients with colorectal cancer undergoing neostomy.To this end,the self-management ability of patients with neostomy should be strengthened,their knowledge of neostomy-related nursing knowledge should be enhanced,and standardized prevention mechanisms should be formulated to reduce the incidence of peripheral dermatitis in patients with colorectal cancer stoma,reduce physical and mental pain,and improve the quality of life after surgery.
