利奈唑胺致血小板减少与二氧化碳分压相关性分析 被引量:3

摘要 目的:研究利奈唑胺致患者血小板减少程度与二氧化碳分压间的相关性。方法:利用解放军总医院药物大数据平台纳入2016年1月1日至2018年12月31日单独或联合使用利奈唑胺后血小板计数下降的47例患者,观察利奈唑胺治疗前、治疗后和停药后血小板计数下降与二氧化碳分压(PaCO2)变化情况并评价其相关性。同时,按血小板计数下降程度分为轻度下降组(n=21)、中度下降组(n=17)和重度下降组(n=9),评价血小板下降程度与合用其他抗菌药物品种数或利奈唑胺使用疗程间的相关性。结果:血小板计数与PaCO2变化存在显著性负相关(r=–0.205,P=0.015),血小板下降程度与治疗前PaCO2值存在显著性正相关(r=0.350,P=0.016),血小板下降程度与患者联用抗菌药物品种数及利奈唑胺疗程无显著性相关。结论:基础PaCO2值可作为预测利奈唑胺致血小板减少发生率的潜在因素之一,PaCO2动态变化可作为判断血小板减少预后的潜在指标之一。 Objective:To study the correlation between the thrombocytopenia caused by linezolid and PaCO2 levels in patients.Methods:A total of 47 patients with thrombocytopenia induced by linezolid from January 1 st,2016 to December 31 st,2018 were selected from drug big data platform in Chinese PLA general hospital.Platelet count,PaCO2 levels and their correlations were observed before using linezolid,after the linezolid treatment and withdrawal.Meanwhile,subjects were also divided into mild-descent group(n=21),mediumdescent group(n=17)and severe-descent group(n=9)according to the degree of thrombocytopenia in order to investigate the correlation between the reduction of platelet count and the number of combined antibiotics or the course of linezolid treatment.Results:There was a significant negative correlation between platelet count and PaCO2(r=–0.205,P=0.015),while there was a significant positive correlation between the degree of thrombocytopenia and the basic PaCO2 level before linezolid treatment(r=0.350,P=0.016).The degree of thrombocytopenia had no significant correlation with the number of combined antibiotics or the course of linezolid treatment.Conclusion:Pa CO2 level before linezolid treatment might be a potential predictor for the occurrence of thrombocytopenia.And the dynamic changes of Pa CO2 level might be used to predict the prognosis of thrombocytopenia caused by linezolid.
