摘要
目的探讨PDCA循环护理管理模式在胃癌根治手术护理中的应用效果及其对并发症发生的影响。方法选取2017年8月至2019年2月本院收治的86例行胃癌根治术的患者为研究对象,按照随机原则将其分成对照组与观察组,各43例。对照组给予常规胃癌根治术围手术期护理,观察组在对照组基础上采用PDCA循环护理管理模式,比较两组的胃肠功能指标、护理前、后3周生活质量及并发症发生情况。结果观察组的术后肠鸣音恢复时间、肛门排气恢复时间、饮食恢复时间及排便恢复时间均明显短于对照组,差异具有统计学意义(P<0.05)。护理前,两组的WHOQOL-BREF评分比较,差异无统计学意义(P>0.05);护理后3周,两组的WHOQOL-BREF评分均显著增高,且观察组高于对照组,差异具有统计学意义(P<0.05)。观察组的胃肠功能障碍、肺部感染、切口感染、吻合口瘘、压疮发生率均明显低于对照组,差异具有统计学意义(P<0.05)。结论PDCA循环护理管理模式可有效促进胃癌患者根治患者胃肠功能恢复,降低并发症发生率,提高患者术后生活质量,值得临床推广。
Objective To investigate the application effect of PDCA circulation nursing management mode in nursing of radical gastrectomy and its influence on the occurrence of complications.Methods From August 2017 to February 2019,86 patients with radical gastrectomy admitted in our hospital were selected as the study objects.According to the random principle,the patients were divided into control group and observation group,with 43 cases in each group.The control group was given perioperative nursing care of conventional radical gastrectomy,and the observation group was given PDCA circulation nursing management mode on the basis of the control group.The gastrointestinal function,quality of life before nursing and three weeks after nursing and complications were compared between the two groups.Results The recovery time of bowel sounds,anal exhaust,diet and defecation in the observation group were significantly shorter than those in the control group,and the differences were statistically significant(P<0.05).Before nursing,there was no significant difference in WHOQOL-BREF score between the two groups(P>0.05);three weeks after nursing,the WHOQOL-BREF score of the two groups increased significantly,and that in the observation group was higher than the control group,the differences were statistically significant(P<0.05).The incidence of gastrointestinal dysfunction,pulmonary infection,incision infection,anastomotic leakage and pressure sore in the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion PDCA circulation nursing management mode can effectively promote the recovery of gastrointestinal function,reduce the incidence of complications,and improve the quality of life of patients after radical gastrectomy,which is worthy of clinical promotion.
