摘要
目的探讨凝血酶联合西咪替丁治疗新生儿上消化道出血的临床效果及对凝血功能的影响。方法选取2018年6月至2019年8月在我院接受治疗的64例上消化道出血新生儿作为研究对象,按双盲法将患儿分为对照组与观察组,每组32例。对照组患儿给予常规治疗;观察组患儿在常规治疗基础上行西咪替丁联合凝血酶治疗。比较两组患儿止血时间、临床症状改善时间、临床疗效、不良反应情况及治疗前、后的凝血功能。结果治疗后,观察组患儿的止血时间、胃管引流无血性液体、大便转黄、大便隐血试验转阴时间均短于对照组(P<0.05)。治疗后,观察组患儿的治疗总有效率高于对照组,不良反应总发生率低于对照组(P<0.05)。治疗后,观察组患儿的FIB、PLT均高于对照组及治疗前,D-D水平、APTT、PT均低于对照组及治疗前(P<0.05)。结论凝血酶联合西咪替丁治疗新生儿上消化道出血可有效提高治疗效果,有效改善患儿的凝血功能,安全性高。
Objective To evaluate the clinical effect of thrombin combined with cimetidine in the treatment of neonatal upper gastrointestinal hemorrhage and its influence on coagulation function.Methods From June 2018 to August 2019,64 neonates with upper gastrointestinal hemorrhage treated in our hospital were selected as the study objects.The children were divided into control group and observation group according to double-blind method,with 32 cases in each group.The control group was given routine treatment,the observation group was treated with cimetidine combined with thrombin on the basis of the routine treatment.The hemostasis time,clinical symptom improvement time,clinical efficacy,adverse reactions and coagulation function before and after treatment were compared between the two groups.Results After treatment,the hemostasis time,time of gastric tube drainage without blood fluid,stool turning yellow and stool occult blood test turning negative in the observation group were shorter than those in the control group(P<0.05).After treatment,the total effective rate of treatment in the observation group was higher than that in the control group,and the total incidence of adverse reactions was lower than that in the control group(P<0.05).After treatment,the levels of FIB and PLT in the observation group were higher than those in the control group and before treatment,D-D level,APTT and PT were lower than those in the control group and before treatment(P<0.05).Conclusion Thrombin combined with cimetidine in the treatment of neonatal upper gastrointestinal hemorrhage can effectively improve the treatment effect,and the coagulation function of the children,with high safety.
