摘要
目的探讨克氏针临时髓内固定联合微创锁定解剖钢板术治疗青少年锁骨骨折的临床效果。方法回顾性分析我院2016年1月至2018年6月收治的80例青少年锁骨骨折患者的临床资料,按治疗方式的不同将患者分为对照组和观察组,各40例。对照组采用切开复位锁定钢板内固定(LCP)治疗,观察组进行克氏针临时髓内固定联合微创锁定解剖钢板治疗。比较两组患者的治疗效果。结果观察组手术时间、出血量、切口长度及骨折愈合时间均优于对照组,差异具有统计学意义(P<0.05)。观察组术后6、12、24 h及3 d的VAS评分均低于对照组,差异具有统计学意义(P<0.05)。术前,两组患者的IL-1、TNF-α水平及Constant-Murley、GQOL-74评分比较,差异无统计学意义(P>0.05);术后,两组患者的IL-1、TNF-α水平及Constant-Murley、GQOL-74评分均改善,且观察组优于对照组,差异具有统计学意义(P<0.05)。观察组术后并发症总发生率显著低于对照组,差异具有统计学意义(P<0.05)。结论克氏针临时髓内固定联合微创锁定解剖钢板治疗青少年锁骨骨折患者的临床效果显著,可减轻炎症反应及术后并发症,促进肩关节功能恢复,值得临床推广。
Objective To explore the clinical effect of Kirschner wire temporary intramedullary fixation combined with minimally invasive locking anatomical plate in teenagers with clavicular fracture.Methods The clinical data of 80 teenagers with clavicular fracture admitted in our hospital from January 2016 to June 2018 were retrospectively analyzed.According to different treatment methods,the patients were divided into control group and observation group,with 40 cases in each group.The control group was treated with open reduction and locking plate internal fixation(LCP),and the observation group was treated with Kirschner wire temporary intramedullary fixation combined with minimally invasive locking anatomical plate.The therapeutic effects of the two groups were compared.Results The operation time,bleeding volume,incision length and fracture healing time of the observation group were better than those of the control group,and the differences were statistically significant(P<0.05).The VAS scores of the observation group at 6,12,24 hours and 3 days after operation were lower than those of the control group,and the differences were statistically significant(P<0.05).Before operation,there were no significant differences in the levels of IL-1,TNF-αand the scores of Constant-Murley and GQOL-74 between the two groups(P>0.05);after operation,the levels of IL-1,TNF-αand the scores of Constant-Murley and GQOL-74 of the two groups improved,and those in the observation group were better than the control group,and the differences were statistically significant(P<0.05).The total incidence of postoperative complications in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Kirschner wire temporary intramedullary fixation combined with minimally invasive locking anatomical plate in teenagers with clavicular fracture has significant clinical effect,it can reduce the inflammatory reaction and postoperative complications,and promote the recovery of shoulder joint function,which is worthy of clinical promotion.
