摘要
目的探究无创正压通气联合氨茶碱治疗老年慢性阻塞性肺疾病合并呼吸衰竭患者的临床效果。方法选择2017年2月至2018年2月我院收治的140例慢性阻塞性肺疾病合并呼吸衰竭患者为观察对象,运用随机数字表法将其分为对照组(n=70,常规治疗)和观察组(n=70,常规治疗+无创正压通气+氨茶碱治疗)。比较两组患者的治疗效果。结果治疗前,两组患者的心率、呼吸频率、pH、PaO2、PaCO2比较,差异无统计学意义(P>0.05);治疗7 d后,两组患者的心率、呼吸频率、pH、PaO2、PaCO2均明显改善,且观察组心率、呼吸频率、PaO2、PaCO2优于对照组,差异有统计学意义(P<0.05)。观察组的治疗总有效率高于对照组,差异有统计学意义(P<0.05)。结论无创正压通气联合氨茶碱治疗老年慢性阻塞性肺疾病合并呼吸衰竭临床效果确切,具有协同作用,值得临床推广应用。
Objective To explore the clinical effect of noninvasive positive pressure ventilation combined with aminophylline on elderly patients with chronic obstructive pulmonary disease and respiratory failure.Methods A total of 140 patients with chronic obstructive pulmonary disease and respiratory failure admitted in our hospital from February 2017 to February 2018 were selected as the observation objects.The patients were divided into control group(n=70,routine treatment)and observation group(n=70,routine treatment+noninvasive positive pressure ventilation+aminophylline treatment)by random number table method.The therapeutic effects of the two groups were compared.Results Before treatment,there were no significant differences in heart rate,respiratory rate,pH,PaO2 and PaCO2 between the two groups(P>0.05);after 7 days of treatment,the heart rate,respiratory rate,pH,PaO2 and PaCO2 of the two groups improved significantly,and the heart rate,respiratory rate,PaO2 and PaCO2 in the observation group were better than the control group,the differences were statistically significant(P<0.05).The total effective rate of treatment in the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).Conclusion Noninvasive positive pressure ventilation combined with aminophylline in the treatment of elderly patients with chronic obstructive pulmonary disease and respiratory failure has a definite clinical effect and synergistic effect,which is worthy of clinical promotion and application.
