我院老年气管切开患者医院获得性肺炎多重耐药菌分布及耐药性特征分析

Analysis of the distribution and drug resistance characteristics of multidrug resistant bacteria in elderly patients with hospital acquired pneumonia after tracheotomy

摘要 目的:分析老年气管切开患者医院获得性肺炎多重耐药菌分布及耐药性的特征。方法:2017年2月-2019年7月选取气管切开后发生获得性肺炎老年患者46例,对所有患者病情资料和耐药性监测结果进行回顾性统计分析。结果:气管切开后发生获得性肺炎的老年患者中检测出大约180株病原菌,通过多重耐药性监测可见,主要耐药菌分布为革兰阴性菌(60%)、真菌(25%)、革兰阳性菌(20%);病原菌的耐药性为40%~70%,耐药性整体较高。结论:气管切开后发生获得性肺炎的老年患者病原菌以革兰阴性菌为主,耐药菌种分布广泛且品种复杂,耐药率高且耐药性强,加上老年患者的身体功能下降,非常容易发生感染,应适当缩短老年患者的住院时间并在日常护理中合理运用抗生素。 Objective:To analyze the distribution and drug resistance characteristics of multidrug resistant bacteria in elderly patients with hospital acquired pneumonia after tracheotomy.Method:46 elderly patients with hospital acquired pneumonia after tracheotomy were selected from February 2017 to July 2019.The data of all patients and drug resistance monitoring results were analyzed retrospectively.Results:About 180 strains of pathogenic bacteria were detected in the elderly patients with acquired pneumonia after tracheotomy,which can be seen through multiple drug resistance monitoring.The main drug-resistant bacteria were Gram-negative bacteria(60%),fungi(25%),Gram-positive bacteria(20%),the drug resistance of pathogenic bacteria was 40%~70%,and the overall drug resistance was high.Conclusion:The main pathogens of acquired pneumonia in the elderly patients after tracheotomy are Gram-negative bacteria.The drug-resistant bacteria are widely distributed and complex,with high drug resistance rate and strong drug resistance.In addition,the physical function of the elderly patients is decreased,and it is very easy to cause infection.We should shorten the hospitalization time of the elderly patients and use antibiotics reasonably in daily care.
