摘要
目的探讨超声引导下治疗性经皮穿刺置管引流术(UG-TPPCD)治疗重症胰腺炎合并胰周脓肿的临床效果。方法选取126例重症胰腺炎合并胰周脓肿患者,随机分为对照组与研究组各63例。对照组采用开腹引流术,研究组采用UG-TPPCD。比较2组疗效。结果研究组总有效率93.65%,显著高于对照组71.43%(P<0.05)。研究组症状、体征缓解时间以及住院时间显著短于对照组,血淀粉酶、尿淀粉酶、白细胞计数(WBC)、C反应蛋白(CRP)显著优于对照组(P<0.05)。研究组并发症发生率6.35%,显著低于对照组19.05%(P<0.05)。结论重症胰腺炎合并胰周脓肿治疗中应用UG-TPPCD效果确切,可改善患者症状、体征,增强疗效,减轻炎性反应,且并发症少。
Objective To investigate the clinical effect of ultrasound-guided therapeutic percutaneous puncture catheter drainage(UG-TPPCD)in the treatment of severe pancreatitis patients complicated with peripancreatic abscess.Methods A total of 126 severe pancreatitis patients complicated with peripancreatic abscess were selected and randomly divided into control group(n=63)and study group(n=63).Open surgery and drainage was used in the control group,while UG-TPPCD was used in the study group.Effects were compared between two groups.Results The total effective rate of the study group was 93.65%,which was significantly higher than 71.43%of the control group(P<0.05).The remission time and hospitalization time of symptoms and signs in the study group were significantly shorter than those in the control group,while the levels of serum amylase,urine amylase,white blood cell count(WBC)and C reaction protein(CRP)in the study group were significantly better than those in the control group(P<0.05).The incidence rate of complications in the study group was 6.35%,which was significantly lower than 19.05%in the control group(P<0.05).Conclusion UG-TPPCD is effective in the treatment of severe pancreatitis patients complicating with peripancreatic abscess,which can improve the symptoms and signs of the patients,enhance the curative effect,relieve the inflammatory reaction and reduce the complications.
