摘要
目的:基于医院病案数据,分析胃癌住院患者住院费用的构成分布情况及其影响因素。方法:选取2017年1月至2018年12月期间在成都市某三甲医院住院治疗且首要诊断为胃癌的全部6 208住院患者的病案数据资料,包括一般人口学资料及住院费用信息,运用描述性统计分析、单因素及多元线性回归方法分析胃癌住院患者住院费用的构成及相关影响因素。结果:64. 6%的胃癌患者的住院费用集中在20 000元及以下区间;胃癌住院患者的平均住院住院费用为26 337. 6元,中位住院费用11 464. 5元。住院费用构成中,前5位的费用顺位依次为手术费、西药费、放射治疗费、麻醉费和检查费。多元回归分析显示,影响胃癌住院患者住院费用的主要因素(按照标准化回归系数绝对值大小排序)依次为:住院天数、医疗费用支付方式、年龄。具体表现为:在控制其他因素的情况下,住院天数越多,住院费用越高;有保险支付患者的住院费用相对于自费患者更高;年龄越大,住院费用越低。结论:手术费、西药费是胃癌患者住院费用的主要构成,住院天数、医疗费用支付方式、年龄是住院费用的主要影响因素。
Objective:To mine the data of medical record system,and analyze the composition and influencing factors of hospitalization expenses of gastric cancer patients.Methods:Medical records of 6,208 inpatients who were diagnosed as gastric cancer and were treated in a level A tertiary general hospital in Chengdu from January 2017 to December 2018 were selected.Data including general demographics and hospitalization expenses were analyzed by descriptive statistical analysis,single factor analysis and multiple linear regression analysis.Results:A total of 64.6%of gastric cancer patients’hospitalization expenses were in the range of 20,000 yuan or less.The average hospitalization expenses of gastric cancer patients were 26,337.6 yuan,and the median were 11,464.5 yuan.Among the expenses,the first five were operation cost,western medicine cost,radiation treatment cost,anesthesia cost and inspection cost(descending order).Multivariate regression analysis showed that the main factors affecting the hospitalization expenses of gastric cancer patients(sorted by absolute values of standardized regression coefficients)were hospitalization days,payment method and age.Specifically,the longer hospitalization days were,the higher hospitalization expenses were,when other factors were constant;hospitalization expenses for patients with insurance were higher than those for patients at their own expense;the older the age,the lower the hospitalization expenses were.Conclusion:Operation costs and western medicine costs are the main components of hospitalization expenses of gastric cancer patients.The main influencing factors of hospitalization expenses are hospitalization days,payment method and age.
