摘要
目的筛选冠心病心绞痛痰瘀互结证中医诊断量表条目,为形成完整量表奠定基础。方法检索中国生物医学文献数据库(CBM)、中国知网(CNKI)、中国期刊数据库(VIP)和PubMed中关于冠心病心绞痛痰瘀互结证的文献,提取文献中冠心病心绞痛所涉及的症状、体征、舌象、脉象特征,结合该领域专家意见,选择高频症状、体征、舌象、脉象形成初步量表。纳入冠心病心绞痛患者112例,采用频数法、t检验法、离散趋势法、相关系数、克朗巴赫系数对初步量表普适性、区分度、敏感性、独立性、代表性进行条目筛选,删除2种以上方法均未选中的条目以形成最终量表。结果结合纳入的118篇文献及专家意见,提取舌质紫暗、脉滑、苔腻、胸闷、舌生瘀点瘀斑、面色晦暗等28个高频条目。采用上述方法筛选后删除纳呆、舌下脉络青紫等10个条目,保留的18个条目分别为胸闷、胸痛、嗜睡、身重、形体肥胖、口黏腻、口唇青紫、面色晦暗、痞满、大便黏滞、舌质紫暗、舌生瘀斑瘀点、苔腻、脉滑、脉弦、脉弦滑、头晕、舌质淡。结论此量表具有普适性高、区分度强、敏感性高、独立性强、代表性好,可为形成具有中医特色的冠心病心绞痛痰瘀互结证中医诊断量表奠定基础。
Objective To select items of traditional Chinese medicine(TCM) diagnostic scale for phlegm-blood stasis syndrome of angina pectoris and coronary heart disease,and to lay a foundation for the formation of a complete scale. Methods China Biology Medicine(CBM),Chinese National Knowledge Infrastructure(CNKI),VIP and PubMed were searched for literatures on syndrome of intermingled phlegm and blood stasis of coronary heart disease and angina pectoris. The symptoms,signs,tongue and pulse characteristics of coronary heart disease and angina pectoris in the literature were extracted,and those with high frequency were selected to form a preliminary scale based on expert opinions in this field. Totally 112 patients with angina pectoris and coronary heart disease were included. Frequency method,t test,discrete trend method,correlation coefficient and Cronbach coefficient were used to select item for universality,distinction,sensitivity,independence,representativeness of the preliminary scale,and delete the items that were not selected by more than 2 methods to form the final scale. Results Combined with the included118 articles and expert opinions,28 high frequency items were extracted,such as dark purple tongue,slippery pulse,greasy fur,tightness of chest,petechiae or ecchymosis on the tongue,dark complexion,etc. With the above selection method,10 items such as anorexia,blue and purple sublingual vein were deleted,and remaining 18 items were chest tightness,chest pain,sleepiness,physical heaviness,obesity,sticky mouth,cyanotic lips,dark complexion,abdominal fullness,viscous stool,dark-purple tongue,petechiae or ecchymosis on the tongue,greasy fur,slippery pulse,wiry pulse,slippery and wiry pulse,dizziness,pale tongue. Conclusion The scale has high universality,distinction,sensitivity,independence and representativeness. It can lay a foundation for the development of TCM diagnostic scale for angina pectoris and coronary heart disease with TCM characteristics.
