多参数磁共振成像对脑胶质母细胞瘤与单发脑转移性肿瘤的鉴别诊断价值

摘要 目的探讨多参数磁共振成像对脑胶质母细胞瘤与单发脑转移性肿瘤的鉴别价值。方法入组59例术前功能磁共振扫描初步判断为单发脑肿瘤患者,其中经术后病理确认为脑胶质母细胞瘤24例,经病理学检查符合脑转移性肿瘤形态、明确系统恶性肿瘤病史的脑转移性肿瘤24例,患者均进行动态对比增强磁共振成像(DCE-MRI)和动态磁敏感对比增强磁共振成像(DSC-MRI)灌注扫描,时间间隔为24 h以上;余11例未经病理学确诊,其中5例进行DSC-MRI扫描,6例进行DCE-MRI扫描。患者进行扫描前,均未进行相关治疗。观察脑胶质母细胞瘤和脑转移性肿瘤间DCE-MRI和DSC-MRI扫描参数对比、鉴别脑胶质母细胞瘤和脑转移性肿瘤的各参数的接受者操作特性曲线(ROC)、信号回复率(PSR)和时间-信号曲线下面积(iAUC)鉴别诊断指标的相关性。结果脑胶质母细胞瘤的平均PSR肿瘤实质区域(TP)高于单发脑转移性肿瘤,差异有统计学意义(P<0.05)。脑胶质母细胞瘤的最小PSR TP高于单发脑转移性肿瘤,差异有统计学意义(P<0.05)。平均PSR TP的敏感性为91.60%,特异性为100.00%,最佳诊断阈值为0.375,AUC为0.923。最小PSR TP敏感性为50.00%,特异性为100.00%,最佳诊断阈值为0.612,AUC为0.785。Pearson相关性分析发现,平均PSR TP和平均iAUC瘤周区域之间呈正相关关系(r=0.591,P=0.003)。结论多参数磁共振成像对脑胶质母细胞瘤与单发脑转移性肿瘤有一定的鉴别诊断价值,术前DSC-MRI及DCE-MRI灌注扫描有助于区分胶质母细胞瘤和单发脑转移性肿瘤。
