摘要
目的探讨肝脏增强CT与MRI检查在肝癌诊断中的应用价值。方法回顾性分析2016年4月至2018年4月在青阳县人民医院接受检查的74例肝癌病例资料,术前患者依次行增强CT和MRI检查,术后行肝脏病理组织学活检。观察两种影像学检查中肿瘤的影像特征。依据术后的病理结果比较增强CT与MRI检查的准确率。结果CT增强扫描时,多数患者在动脉期病灶强化明显但不均匀;门静脉期病灶与周围正常肝组织密度统一,强化程度减低;延迟期影像呈稍低密度影。MRI增强扫描时,动脉期强化显著,早期肝实质内可见弥漫分布大小不等结节样高信号,门静脉期为等信号,延迟期为较低信号,周围包膜强化。对照病理结果,使用MRI检查的准确率为94.6%(70/74),明显高于使用增强CT的准确率(81.1%,60/74),差异有统计学意义(P<0.05)。结论使用MRI诊断肝癌,相较于增强CT具有更高的准确率。
Objective To explore the application value of liver enhanced computed tomography(CT)and liver magnetic resonance imaging(MRI)in the diagnosis of liver cancer.Methods Data of 74 cases of liver cancer examined in qingyang county people's hospital from April 2016 to April 2018 were retrospectively analyzed.All the patients underwent enhanced CT and MRI examinations successively before surgery,and postoperative liver histopathological biopsy.The imaging features of tumor in the two imaging examinations were observed.The coincidence rate of enhanced CT and MRI was compared according to postoperative pathological results.Results On enhanced CT,most of the patients had obvious but uneven enhancement in arterial lesions,the portal venous stage lesion was consistent with the surrounding normal liver tissue density,and the degree of reinforcement decreased;the delayed images showed slightly lower density shadows.During the enhanced MRI,the enhancement in the arterial phase was significant.Early hepatic parenchyma can be seen with diffuse distribution of nodular hyperintensity of varying sizes,the portal vein stage is equisignal,the delay period was lower signal and the surrounding capsule was strengthened.The coincidence rate of pathological results of MRI examination was 94.6%(70/74),significantly higher than that of enhanced CT examination(81.1%,60/74).The difference between groups was statistically significant(P>0.05).Conclusion MRI has a higher accuracy to diagnose liver cancer compared with enhanced CT.
