摘要
目的观察分析气道感染的急性期慢性阻滞性肺疾病(COPD)患者外周血CD8^+CD28-T的细胞比例及患者血清白介素-10(IL-10)浓度的变化及意义。方法选取180例COPD急性期病例作为观察组,并选择同期本院健康体检者50例为对照组。采用五分类法测血常规,流式细胞术(FCM)检测CD8^+CD28-T细胞在不同气道感染中的比例。化学发光法检测IL-10浓度,生化分析法检测CRP浓度,并对两组患者进行痰培养,分析对比两组检验结果。结果与健康对照组相比,3种炎症指标(白细胞量、中性粒细胞百分比、CRP浓度)、CD8^+CD28-T细胞比例和IL-10浓度在急性期COPD患者中显著增加(P<0.01)。结论COPD急性加重期患者CD8^+CD28-T细胞与IL-10浓度水平均增加,比例呈正相关,提示在气道感染COPD患者中两者均对T淋巴细胞起抑制作用,临床可通过免疫调节剂或免疫抑制剂降低CD8^+CD28-T细胞与IL-10浓度水平的方式来治疗COPD患者。
Objective To observe the ratio of CD8^+CD28-T cells in peripheral blood of patients with chronic obstructive pulmonary disease(COPD)and the changes and significance of serum interleukin-10(IL-10)concentration in patients with acute airway infection.Methods One hundred and eighty patients with acute COPD were selected as observation group,and 50 healthy persons were selected as control group.The percentage of CD8^+CD28-T cells was detected by flow cytometry(FCM),the concentration of IL-10 was detected by chemiluminescence,the concentration of CRP was detected by biochemical analysis,and the sputum culture was carried out in both groups.The results of the two groups were analyzed and compared.Results Compared with the healthy control group,the three inflammatory markers(white blood cell count,neutrophil percentage,and CRP concentration),CD8^+CD28-T cell ratio and IL-10 concentration were significantly increased in patients with acute COPD(P<0.01).Conclusion The percentage of CD8^+CD28-T cells in peripheral blood of patients with acute COPD increases,which is positively correlated with the level of IL-10.
