摘要
目的探讨肺癌并糖尿病围手术期病人胰岛素泵的应用效果,为制定病人的血糖管控方案提供科学依据。方法选择90例行肺癌切除术的糖尿病病人作为研究对象,随机分为两组,各45例。实验组采用胰岛素泵注射胰岛素,对照组采用传统多次皮下注射胰岛素。出院时,对两组病人满意度、生存质量、血糖达标时间、糖化血红蛋白值、住院时间和住院费用等指标进行评估。结果实验组血糖达标时间短于对照组,每日平均胰岛素剂量少于对照组,平均住院时间短于对照组,差异有显著性(t=8.89~15.77,P<0.05);两组住院费用比较差异无显著性(P>0.05)。两组病人生存质量指标中躯体活动功能、疼痛、社会功能和一般健康状况维度得分差异有统计学意义(t=2.43~3.78,P<0.05)。结论与多次皮下注射胰岛素比较,肺癌并糖尿病病人围手术期采用多学科协作管控下持续胰岛素泵皮下输注胰岛素控制血糖更安全、有效。
Objective To explore the application effect of insulin pump in patients with lung cancer and diabetes mellitus during the perioperative period,and to provide a scientific basis for blood glucose control.Methods A total of 90 patients with diabetes mellitus who underwent lung cancer resection were randomly divided into two groups,with 45 cases in each group.The experimental group was injected with insulin using an insulin pump,while the control group was treated with conventional multiple subcutaneous injections of insulin.At the time of discharge,the two groups were compared regarding patients’degree of satisfaction,quality of life,time to reach target blood glucose,glycosylated hemoglobin value,length of hospital stay,and hospital costs.Results The experimental group had a significantly shorter time to reach target blood glucose,a significantly lower mean daily dose of insulin,and a significantly shorter mean length of hospital stay compared with the control group(t=8.89-15.77,P<0.05).However,there was no significant difference in hospital costs between the two groups(P>0.05).There were significant differences in the scores of physical activity function,pain,social functioning,and general health status between the two groups(t=2.43-3.78,P<0.05).Conclusion With multidisciplinary cooperation,continuous subcutaneous insulin pump is safer and more effective in controlling blood glucose in patients with lung cancer and diabetes mellitus during the perioperative period compared with multiple subcutaneous insulin injections.
