摘要
目的评估中国老年HIV感染者睡眠障碍的患病率和相关危险因素。方法选取2014年1月至2015年12月全国20个HIV抗病毒治疗临床医院或疾控中心的641例老年HIV感染者进行横断面研究。匹兹堡睡眠质量指数(Pittsburgh sleep quality index,PSQI)由受试者自主报告。收集社会人口统计学特征、病史和HIV相关临床数据。结果641例老年HIV感染者数据完整,47.1%有睡眠障碍。多因素logistic回归分析显示,相关危险因素包括:心理因素,抑郁(OR=1.876,95%CI:1.234~2.852,P<0.003)、焦虑(OR=1.688,95%CI:1.036~2.752,P=0.036);社会人口学因素,同性性传播(OR=1.896,95%CI:1.099~3.270,P=0.022)、吸毒(OR=2.456,95%CI:1.025~5.886,P=0.044);临床因素,服用依非韦伦(EFV)(OR=11.302,95%CI:2.565~49.799,P=0.001)。结论中国老年HIV感染者睡眠障碍的患病率很高,与心理、社会人口与临床药物等因素相关。卫生工作者应考虑定期评估老年HIV感染患者的睡眠障碍,并提供及时的精神卫生服务,这将对治疗依从性产生积极影响。
Objective To assessed the prevalence and related risk factors of sleep disorders in elderly HIV-infected patients in China.Methods Cross-sectional studies were performed in HIV-infected patients with antiretroviral therapy at 20 AIDS clinics.The Pittsburgh sleep quality index(PSQI)was reported by the subject.Social demographic characteristics,medical history,and HIV-related clinical data were collected.Results Six hundred and forty-one elderly patients had complete data.Sleep disorders were observed in 47.1%of elderly HIV patients.Relevant risk factors in multivariate analysis included psychological factors:depression(OR:1.876,95%CI:1.234-2.852,P<0.003),anxiety(OR:1.688;95%CI:1.036-2.752,P=0.036);sociodemographic factors:homosexual transmission(OR:1.896;95%CI:1.099-3.27,P=0.022),drug use(OR:2.456;95%CI:1.025-5.886,P=0.044);and clinical factors:taking efavirenz(EFV)(OR:11.302;95%CI:2.565-49.799,P=0.001).Conclusions The prevalence of sleep disorders among elderly people living with HIV in China is very high,which is related to factors such as psychology,social population and clinical medicines.Health workers should consider regularly assessing sleep disorders in elderly patients with HIV infection and provide timely mental health services,which will have a positive impact on treatment compliance.
