摘要
目的评价改良单孔逐步加压冲洗引流治疗慢性硬膜下血肿(CSDH)的效果及安全性。方法选入2018年1月-2019年12月在博州人民医院神经外科接受改良单孔逐步加压冲洗引流治疗的50例CSDH患者作为观察组,2018年1月前采用传统钻孔引流术治疗的50例作为对照组。评价两组的治疗效果、术后恢复、并发症等指标,并进行统计比较。结果两组手术时间、切口长度、术中出血量、临床疗效和各种并发症发生率均无明显差异(P>0.05);观察组术后第1d引流量显著大于对照组,术后第1d硬膜下积液量和内积气量显著低于对照组(P<0.05);而观察组住院天数以及复发率显著低于对照组(P<0.05)。结论与传统钻孔引流术治疗CSDH相比,改良单孔逐步加压冲洗引流在缩短住院时间及复发率方面更具优势。
Objective To evaluate the efficacy and safety of improved single-hole progressive pressure flushing and drainage in the treatment of chronic subdural hematoma(CSDH).Methods Fifty patients with CSDH who received improved single-hole progressive pressure flushing and drainage from January 2018 to December 2019 in department of neurosurgery of people's hospital at Bozhou were selected as observation group,and 50 patients who were received traditional drilling and drainage before January 2018 were chosen as control group.The therapeutic effect,postoperative recovery and complications were compared between the two groups.Results There were no significant differences in operation time,incision length,intraoperative bleeding volume,clinical efficacy and incidence of complications between the two groups(P>0.05).The drainage volume on the first day after operation in the observation group was significantly higher than that in the control group,and the volume of subdural effusion and gas accumulation on the first day after operation in the observation group was significantly lower than those in the control group(P<0.05).While the hospital stay and recurrence rate in the observation group were significantly lower than those in the control group(P<0.05).Conclusion Compared with traditional drilling drainage in the treatment of CSDH,the improved single-hole progressive pressurized flushing and drainage has more advantages in shortening hospitalization time and recurrence rate.
