摘要
目的通过观察热性惊厥患儿血浆中神经肽B(NPB)、神经肽Y(NPY)、甘丙肽(GAL)水平,探讨NPB、NPY、GAL在热性惊厥发病中的作用。方法选取热性惊厥患儿(6个月~6岁)50例为实验组[其中单纯性热性惊厥(SFS)组36例,复杂性热性惊厥(CFS)组14例],同时选自本院同期住院期间发热但未发生惊厥的40例患儿为观察组,门诊正常体检儿童40例为对照组,采用酶联免疫吸附法和放射免疫分析法检测血浆中NPB、NPY、GAL水平。 结果(1)热性惊厥后第1天实验组NPB、NPY、GAL水平均明显上升,与观察组、对照组比较,差异有统计学意义(P<0.05);观察组NPB、NPY和GAL水平略高于对照组,但差异无统计学意义(P>0.05)。(2)热性惊厥后第7天实验组NPY、GAL水平仍明显升高,与观察组比较,差异有统计学意义(P<0.01),而实验组NPB水平较前下降,与同期观察组比较,差异无统计学意义(P>0.05),与热性惊厥后第1天比较差异有统计学意义(P<0.01)。(3)惊厥后第1天CFS组患儿NPB、NPY和GAL水平明显高于SFS组患儿,差异均有统计学意义(P<0.05)。(4)惊厥后第7天CFS组患儿NPY和GAL水平明显高于SFS患儿,差异有统计学意义(P<0.05)。结论热性惊厥发作后第1天,患儿血浆中NPB、NPY、GAL水平增高;热性惊厥发作第7天时,NPY、GAL水平仍增高,而NPB水平下降,NPB、NPY、GAL水平增高程度与惊厥发作程度及惊厥发作次数相关,表明NPB、NPY、GAL可能与热性惊厥的发病及病情进展有关。
Objective To investigate the roles of neuropeptide B(NPB)、neuropeptide Y(NPY)and galanin(GAL)in the pathogenesis of febrile seizure through observing the levels of NPB,GAL in plasma. Methods Fifty child patients(6 months-6 years old)with febrile seizures were selected as the experimental group,of whom 36 were in the SFS group with simple febrile seizures,14 were in the CFS group with complex febrile seizures.At the same time,40 child inpatients with fever but without convulsion were designated as the observation group.Another 40 child patients who underwent normal physical examination were assigned as the control group.Enzyme linked immunosorbent assay(ELISA)and radioimmunoassay(RIA)were used to detect serum levels of NPB,NPY and GAL. Results (1)NPB,NPY and GAL levels on the first day after convulsion in the experimental group all increased significantly,and statistical significance could be noted,as compared with those of the observation group and the control group(P<0.05).NPB,NPY and GAL levels in the observation group were slightly higher than those of the control group,but without statistical significance(P<0.05).(2)NPY and GAL levels on the 7 th day after convulsion in the experimental group still elevated obviously,and statistical significance could also be found,as compared with those of the observation group(P<0.01),while NPB level in the experimental group decreased as compared with the previous level,and there was no statistical significance,as compared with that of the observation group within the same time span(P<0.05).However,there was statistical significance,as compared with the convulsion that occurred on the first day(P<0.01).(3)NPB、NPY and GAL levels on the first day after convulsion were significantly higher than those of the CFS group,and statistical significance could be noticed,when comparison were made between them(P<0.05).(4)NPY and GAL levels in the CFS group on the 7 th day after convulsion were significantly higher than those of the SFS group,with statistical significance(P<0.05).NPB concentration level of the CFS group was slightly lower than that of the SFS group,but without statistical significance(P<0.05). Conclusion The levels of NPB,NPY and GAL in the child patients on the first day after febrile seizure elevated,and NPY and GAL levels on the 7 th day after convulsion still increased,while NPB level decreased.The increased levels of NPB,NPY and GAL were associated with the number of convulsion onset,indicating that NPB,NPY and GAL levels might be related with the onset and progress of febrile seizures.
