摘要
目的探究苗药五藤贴膏外敷联合艾灸治疗早中期膝骨关节炎(KOA)的临床疗效。方法将符合纳入标准的轻中度KOA患者84例,按随机数字表法将其分为治疗组和对照组。治疗组42例予苗药五藤贴膏外敷联合艾灸治疗,对照组42例予非甾体类抗炎药塞来昔布口服,连续治疗2 w。治疗前后采用骨关节炎指数量表(WOMAC)评分系统评估KOA严重程度,视觉模拟评分法(VAS)评估膝关节疼痛程度及奎森功能演算指数(Lequesne评分)评价膝关节功能;采用酶联免疫吸附试验(ELISA)检测超敏C反应蛋白(hs-CRP)、白细胞介素(IL)-1并评价两组临床疗效。结果两组总有效率差异有统计学意义(P<0.05)。治疗2 w后,两组VAS评分、WOMAC量表评分及Lequesne各项评分、血清hs-CRP、IL-1均较治疗前显著改善(P<0.05,P<0.01),且治疗组改善更显著(P<0.05)。结论采用苗药五藤贴膏外敷联合艾灸治疗早中期KOA能显著改善轻中度KOA患者的膝关节功能,减轻关节疼痛,有效控制炎症反应。
Objective To explore the clinical effect of external application of Miao medicine wuteng plaster combined with moxibustion on the treatment of early and middle stage knee osteoarthritis.Methods Eighty-four patients with mild to moderate knee osteoarthritis(KOA)who met the inclusion criteria were divided into two groups treatment and control groups according to the random number table.Forty-two patients in treatment group were treated with Miao medicine wuteng plaster combined with moxibustion,while 42 patients in control group were treated with non-steroidal anti-inflammatory drug celecoxib orally.KOA severity was assessed by the Western Ontario McMaster Universities(WOMAC)scoring system before and after treatment,and knee function was assessed by visual analogue scoring(VAS)and Lequesne functional rating(Lequesne).Hypersensitive C-reactive protein(hs-CRP)and interleukin(IL)-1 were detected by ELISA to evaluate the clinical efficacy of KOA patients in 2 groups.Results In the treatment group,there were 15 cases of clinical control,21 cases of obvious effect,3 cases of effective,3 cases of ineffective,and the total effective rate was 92.85%;in control group,there was 10,10,8,14 cases,respectively,and the total effective rate of 66.67%,the difference was statistically significant(P<0.05).Two weeks after treatment,VAS,WOMAC scale and Lequesne scores,serum levels of hs-CRP and IL-1 in both groups were improved compared with those before treatment(P<0.05 or P<0.01),but the improvement was more significant in treatment group.Conclusions The external application of Miao medicine wuteng plaster combined with moxibustion on the treatment of early-middle stage patients with KOA could significantly improve the knee joint function of patients with mild to moderate knee osteoarthritis,reduce the pain of patients with joint,and effectively control the inflammatory response.
