摘要
目的 探讨HCC病灶在DCE MRI增强表现与微血管密度之间的关系。方法 经手术病理证实的HCC患者 3 0例 ,按MRI动脉期图像信号高、等、低部位取材 67处 ,进行HE染色和CD3 4免疫组化染色。每位患者术前均行SET1 W ,FSET2 W及FMPSPGR三个序列常规平扫成像 ,静注Gd DTPA后 ,采用FMPSPGR序列进行DCE MRI扫描 ,评价动脉期强化程度、静脉期强化及平衡期强化形式 ,并计算代表强化程度和形式的病灶强化指数和肝脏相对增强比值 (RE % ) =(S增强后信号强度 S增强前信号强度 ) S增强前信号强度× 10 0 % ,与MVD的结果对照分析。结果 动脉期图像中明显强化组MVD值 (2 2 9.79± 80 .96) ,明显高于中等强化组 (173 .0 9± 61.3 8)和低度强度或无强化组 (15 3 .0 0± 10 8.5 8) (P <0 .0 5 ) ;静脉期强化组MVD值为 2 5 9.80± 93 .3 ,高于无强化组 (178.64± 92 .65 ) (P <0 .0 5 ) ;延迟期出现无环状强化和环状强化组中 ,MVD值分别为 14 4.10± 5 3 .2 4和 2 69.0 6± 95 .0 8,MVD值与强化指数有明显的相关性 (相关系数为 0 .771,P <0 .0 1)。
Objective To study the correlation between the enhancement patterns on dynamic contrast enhancement magnetic resonance imaging(DCE MRI) and tumor microvessel density(MVD).Method MRI examination was performed with GE Signa 1.5T MR scanner using spin echo and FMPSPGR sequence in 30 cases(48 lesions) of HCC confirmed with histology.SET 1 weighted imaging,proton density weighted imaging,and T 2 weighted imaging were performed with cross section,FMPSPGR also done with the same position.The DCE MRI was done after the precontrast scanning at the arterial,portal vein and delay phase using FMPSPGR sequence.Specimens were stained immunohistochemically for CD34 and MVD was highlighted by anti CD34.The enhancement imaging features of HCC lesions were studied correlatively with tumor MVD.Result MVD was higher in the group of high enhancement(229.79±80.96) than that of medium enhancement(173.09±61.38) and slight enhancement(153.00±108.58)(P<0.05) at the arterial phase.MVD was higher in the group of enhancement(259.80+93.3) than that of non enhancement(153.00±108.58)(P<0.05) at the protal vein phase.At delay phase,MVD was significantly higher in the group of ringed enhancement(269.06±92.08) than that of without ringed enhancement(144.10±53.24)(P<0.05).RE(relative enhancement)percentage had correlation with MVD(r=0.771,P<0.01).Conclusion The enhancement patterns on DCE MRI are influenced by MVD in HCC lesions.
