摘要
目的 评价增视六联法治疗儿童弱视的疗效。探讨影响弱视疗效的因素。方法 对 8～ 12岁 4 6 3例弱视儿童使用增视六联法的疗效进行 3年以上的追踪观察 ,并进行疗效比较分析。结果 总治愈率达 84 .98%。弱视程度与疗效比较 ,轻度 97.6 6 % ,中度 87.5 7% ,重度 5 8.4 7% ,3组间疗效差异有极显著性 (P <0 .0 1)。弱视程度越重 ,疗效越差。注视性质与疗效比较 ,5组间差异有高度显著性 (P <0 .0 1)。中心和不稳定中心注视性弱视治愈率最高 (97.4 % ,93.94 % ) ,两组间差异无显著性 (P >0 .0 5 )。旁黄斑性和周边注视性弱视治愈率最低(10 .34% ,11.11% ) ,两组间差异也无显著性 (P >0 .0 5 )。旁中心注视弱视治愈率达 6 5 .87%。其中重度弱视治愈率 6 3% ,与旁黄斑和周边注视两组间疗效差异有高度显著性 (P <0 .0 1)。说明旁中心注视中的重度弱视是有治愈可能性的。弱视未愈原因调查中发现不能配合坚持治疗的弱视 31眼占未愈总数 31.31%。 8～ 12岁大龄单眼弱视学龄儿童或高度近视、高度散光引起的单眼屈光参差性弱视共有 2 6眼 ,占 2 6 .3%。结论 增视六联法治疗儿童弱视 ,进一步证实有明显的增效作用。影响儿童弱视疗效的因素有弱视程度 ,注视性质 ,弱视类型 ,年龄 ,单眼高度近视。
Objective\ To evaluate the effect of pleoptics combined with six other methods and study some factors affecting the effect on amblyopia.Methods\ Observing and comparing the effects of the pleoptics combined with six other methods on 463 children with amblyopia for over 3 years.Results\ The total recovery rate was 84.98%,that of minimal degree of amblyopic was 97.66%,moderate degree,87.57%,severe degree,58. 47%.The difference was very significant among the three groups(P<0.01).The effect depends on the patients' conditions .The severe the worse.That of central fixation of amblyopia was 97.4%,unstable central fixation, 93.94%.The recovery rate of two groups was the best,There was no significant difference between two groups.That of ecmacular fixation of amblyopia was 10.34%,peripheral fixation,11.11%,the recovery rate of two groups was the worst,There was no signficant difference between two groups.That of the eccentric fixation of amblyopia was 65.87%,of severe degree,63%.The difference was higher than ecmacular and peripharal, which showed that the severe degree of ambtlyopia in eccentric fixation could be cured.Those that can't be cured in the childhood amblyopia are the patients with high miopia, high astigmatism,older age and these patients can't cooperate very well with doctors during the treatment.Conclusion\ The pleoptics combined with six methods confirms its efficiency.Factors affecting the effect of amblyopia therapy were the degree of amblyopia, The property of focus,type of amblyopia,monocular high myopia or high astigmatism,age of onset and some man mode elements.
