摘要
目的 :评价美托洛尔治疗陈旧性心肌梗死合并慢性心力衰竭的有效性和安全性。方法 :45例陈旧性心肌梗死合并慢性心力衰竭 (NYHA心功能Ⅱ～Ⅲ级 )病人在我院门诊接受治疗 ,所有病人常规应用利尿剂、ACEI、或地高辛等药物 ,病情稳定后随机分两组 :治疗组 2 2例 ,对照组 2 3例。治疗组加用美托洛尔 ,从 6 .2 5mg Bid开始 ,2周后加量至 12 .5mg Bid ,再 2周后加量至目标剂量 2 5mg Bid。有效定义为临床症状好转、NYHA心功能改善Ⅰ级或以上。平均随访时间 1年。结果 :美托洛尔治疗组有效 17例 ,有效率 77.3% ;对照组有效 11例 ,有效率 4 7.8% ,两组比较有效率有显著性差别 (P<0 .0 5 )。两组死亡率、再次入院率比较无显著性差别 (P >0 .0 5 )。治疗组无 1例发生低血糖反应、心动过缓、房室传导阻滞。结论 :美托洛尔治疗陈旧性心肌梗死合并慢性心力衰竭有效且安全。
Objective:To evaluate the efficacy and safety of metoprolol in the treatment of chronic heart failure due to old myocardial infarction. Methods: 45 patients with chronic heart failure due to old myocardial infarction, NYHA functional classification II-III, were followed up in out-patient department. All patients with symptomatic left ventricular dysfunction were routinely managed with a combination of 3 types of drugs: a diuretic, an ACE inhibitor, and (usually) digitalis. 45 stable patients were randomizedly divided into two groups: 22 patients on metoprolol treatment and 23 patients as control. Metoprolol: was initiated as 6.25mg PO Bid, two weeks later 12.5mg PO Bid, 25mg PO Bid as target dose four weeks later. Effectiveness was defined as symptomatic relief and improvement in NYHA functional classification. The median period of follow-up is 1 year. Results: 17 of the 22 patients (77.3%) in metoprolol-treated group indicated symptomatic relief and improvement in NYHA functional classification, 11 of the 23 patients (47.8%) in control. There was significant difference in effectiveness between metoprolol-treated group and control group (p<0.05). Mortality and re-hospitalization was the same for both groups (p>0.05). No occurrence of hypoglycemia, bradycardia, AV block in metoprolol-treated group. Conclusions: Metoprolol is effective and safe in the treatment of chronic heart failure due to old myocardial infarction.
