摘要
从1987年12月～1990年12月施行肝段切除28例,男22例,女6例,年龄21～68岁,中位年龄48岁。原发性肝癌15例,转移性肝癌2例,肝血管瘤4例,肝内胆管结石4例,其它3例,采用Kelly钳和CUSA断离肝组织,无手术死亡和严重并发症。根据肝表面标志,借助术中B超及CUSA施行肝段切除比较安全、出血少,可保留较多正常肝组织。
From December 1987 to December 1990, 28 hepatic segmentectomies including 22 males and 6 females, age 21 to 68 years with median age of 48 years old. Fifteen cases were primary hepato carcinoma, two metastatic tumors, four hemangioma, four hepatoligthiasis and three miscellaneous diseases. The simple technique using Kelly clamp and a new technique using the Caritron Ultrasonic Surgical Aspiration (CUSA) were practised. No death or complications were encountered. Observing the surface anatomy of the liver and with the aid of intraoperative ultrasound scanning, single or multiple segmentomies can be safely performed. Greater amount of normal liver tissue may be preserved with less bloodloss. The advantages of dissection with CUSA was also discussed.
