肝移植对肝硬化大鼠胃运动功能的影响 被引量:2

Effects of liver transplantation on gastric motor of hepatic cirrhosis rats

摘要 目的 主要了解肝移植前后胃运动功能的变化情况以及与胃运动相关的胃肠道激素或神经肽的变化 .方法 制备 CCl4中毒性肝硬化大鼠模型 ,采用“二袖套法”进行肝移植 ,应用放免分析法 ,以 SPECT显像用的核素 (99Tcm )为标记 ,采取液相胃排空的方法 ,分析肝移植前后大鼠胃排空的情况 ,以及与胃运动有关的胃肠道激素或神经肽的变化 .结果 肝移植前 ,肝硬化大鼠的胃排空率明显降低 [胃排空率从(89.3± 6 .8) %至 (78.2± 6 .5 ) % ,P<0 .0 1],与胃运动相关的胃肠道激素或神经肽水平也有所改变 [胃动素从 (98± 6 )ng· L- 1 至 (5 2± 5 ) ng· L- 1 ,P<0 .0 1,抑胃肽从 (5 4± 4 )ng· L- 1至 (88± 3) ng· L- 1 ,P<0 .0 1,生长抑素从 (2 18±16 ) ng· L- 1 至 (2 84± 12 ) ng· L- 1 ,P<0 .0 1,降钙素基因相关肽从 (134± 18) ng· L- 1至 (184± 2 2 ) ng·L- 1 ,P<0 .0 1].肝移植后 ,随着时间的不断延长 ,大鼠胃排空率明显升高 [胃排空率从 (78.2± 6 .5 ) %至 (88.1± 6 .7) % ,P<0 .0 1],与胃运动相关的胃肠道激素或神经肽水平也发生了相应的变化 [胃动素从 (5 2± 5 ) ng· L- 1 至 (95± 6 ) ng· L- 1 ,P<0 .0 1,抑胃肽从 (88± 3) ng· L- 1 至 (5 6± 6 ) ng· L- 1 ,P<0 .0 1,生长抑素从 AIM To observe the changes of rats gastric motor and the changes of plasma motilin (MTL), gastric inhibitory peptide (GIP), somatostatin (SS) and calcitonin gene related peptide (CGRP) before and after liver transplantation. METHODS Hepatic cirrhosis model was established in rats by subcutaneous injections of 40% carbon tetrachloride. Rat liver transplantation model was made with two cuff technique. Nuclide 99 Tc m and gastric emptying of liquids were used before and after liver transplantation. By radioimmunoassay method, the changes of plasma motilin (MTL), gastric inhibitory peptide (GIP), somatostatin (SS) and calcitonin gene related peptide (CGRP) were observed before and after liver transplantation. RESULTS The gastric emptying rate was decreased [the gastric emptying rate from ( 89.3 ±6.8)% to (78.2±6.5)%, P <0.01], the plasma level of MTL was decreased [from (98±6) ng·L -1 to (52±5) ng·L -1 , P <0.01], the plasma levels of GIP [from (54±4) ng·L -1 to (88±3) ng·L -1 , P <0.01], SS [from (218±16) ng·L -1 to (284±12) ng·L -1 , P <0.01] and CGRP [from (134±18) ng·L -1 to (184±22) ng·L -1 , P < 0.01] were increased gradually before liver transplantation. After transplantation, the gastric emptying rate was increased significantly [the gastric emptying rate from (78.2±6.5)% to (88.1±6.7)%, P <0.01] and the plasma level of MTL was increased [from (52±5) ng·L -1 to (95±6) ng·L -1 , P <0.01], the plasma levels of GIP [from (88±3) ng·L -1 to (56±6) ng·L -1 , P <0.01], SS [from (284±12) ng·L -1 to (222±15) ng·L -1 , P <0.01] and CGRP [from (184±22) ng·L -1 to (145±12) ng·L -1 , P <0.01] were decreased gradually. CONCLUSION Liver transplantation can improve gastric motor and gastric emptying. This function may be associated with its effect on some gut and neuropeptid.
