摘要
目的 :提高对艾滋病合并肺结核的认识 ,探讨艾滋病与肺结核的相关性。方法 :回顾性分析 3 2例艾滋病合并症———肺结核的临床与X线表现 ,结合文献进行讨论。结果 :3 2例中血行播散型肺结核 17例 ,继发型 15例。 12例在确诊HIV感染和AIDS前已患肺结核 ,其病灶仅局限某一叶 ,抗结核治疗 5个月后见病变均有不同程度吸收好转。当血清内发现HIV感染和确定AIDS后 ,继续用同样抗结核治疗 1～ 5个月后 ,却见结核灶扩散到二叶以上。 2 0例先确定HIV感染后患肺结核者 ,抗结核治疗 3～ 10个月后 ,未见结核灶吸收 ,反而见病变在发展 ,该 2 0例也在三个月内快速发展为AIDS。结论 :HIV感染和AIDS易并发肺结核和加速结核病发展 ,结核病也可促使HIV感染发展AIDS。
Objective:To improve the knowledge of AIDS with concurrent pulmonary tuberculosis,and study their correlation.Methods:The clinical and X ray manifestations of 32 cases of AIDS accompanying with pulmonary tuberculosis were retrospectively analyzed and discussed.Results:In 32 cases of AIDS with concurrent pulmonary tuberculosis,17 cases were hematogenous tuberculosis and 15 were secondary tuberculosis of lung.12 cases with negative HIV infection suffered from tuberculosis of lung with lesions in one lobe,were treated for 1～5 months and absorbed in varying degrees.However,the lesions with the same treatment mentioned previously for 1～5 months,spread into two lobes or more after HIV infection became positive.20 cases with positive HIV infection were infected pulmonary tuberculosis later and treated over 3～5 months for TB.The TB foci of those patients were not absorbed and even became worse.And also those 20 patients rapidly developed AIDS in 3 months.Conclusion:The patient with HIV infection and AIDS is susceptible to the concurrent tuberculosis of lungs.HIV infection and AIDS may make the tuberculosis of lungs worse while tuberculosis of lungs makes HIV infection to develop AIDS.
