摘要
目的研究白介素-1α(IL-1α)在退变腰椎间盘中的表达与分布,探讨其在坐骨神经痛发生机制中的作用。方法收集30例腰椎间盘突出症手术切除的椎间盘组织,每例在3个部位取材:(1)凸入椎管内与神经根紧贴的椎间盘组织(A组);(2)软骨终板间的椎间盘(B组);(3)软骨终板(C组)。6份正常髓核取自4例急性腰椎外伤骨折患者。应用IL-1α单克隆技术,采用SP免疫组化染色技术,对标本中IL-1α表达水平进行检测。结果IL-1α表达水平A组明显高于B组及C组(P<0.05)。结论IL-1α在退变腰椎间盘3个部位均有表达,但分别不一致;腰椎间盘突出症术后坐骨神经痛不能缓解,可能与残留椎间盘中IL-1α有关。
Objective To study the distribution of IL-1αin different parts of the herniated i ntervertebral disc,and to determin e the role of IL-1αin the mechanism of sciatic pain in lu mbar disc herniation.Methods All of samples of herniated lumbar di scs were obtained from 30adult patients operated on for prima ry translaminar microendscopic dis cectomy (MED)because of persistent radicular sym ptoms and subdivided into three samples:(1)The nearest to the nerve root in close contact with it(A group),(2)The sample obtained from the intervertebral space(B group),(3)The sample obtained from the vertebr al plates(C group).In the present study,the specimens were analyzed immunohisto-chemically using monoclonal murine antibody against human IL-1αfor the presence of IL-1α.As a control group,6lumbar disc specimens were harvested from 4patients suffering from acute bursting fracture of lumb ar spine were excised and analyzed by the same procedure.Results Of 3o herniated lumbar discs,IL-1αpositively expressing cells were pr esented in 25samples in the A group.I n the C group,IL-1αpositively,expressing cells were presented in 21samples.These staining cells were mainly chondrocyte and fibroblast.No positively expressing cells were detected in the normal lumbar discs.IL-1αpositively expressing cells of the A group were most in 3group(P<0.05).Conclusion Herniated lumbar disc cells can prod uce IL-1α.The distribution of IL-1αwas not consistent in different parts of the herniated intervertebral disc.IL-1αmay play an important role in the mech anism of sciatic pain.
