摘要
对109例肺癌患者行纤支镜、支气管肺泡灌洗液(BALF)及外周血肿瘤标记物检测,结果:肺癌患者BALF及外周血中肿瘤标记物CEA、SIgA、SF和β_2—MG检测结果显示,BALF及外周血中SF和β_2—MG对肺癌诊断价值不大(P>0.05),而外周血CEA、病侧BALF中及患侧与健侧BALF中的CEA和SIgA的差值则有助于肺癌的诊断。病侧BALF中的CEA和SIgA联合检测能提高肺癌的诊断率。本文单纯纤支镜活检合并刷检诊断肺癌的阳性率为76.2%,结合病侧BALF中CEA和SIgA检测,诊断肺癌阳性率提高到98.1%。
We reported the results of FB and tumor marks in BALF and. PB examinations. If neoplasm and/or tube narrow were watched in FB,diagnosing rate(94.1%) was higher in jointed examinations of biopsy and brush, and biopsy diagnosing rate(76.5%) was higher than brush(30.9%). If mucosal cogestion, edema, erosion or normal in watching from FB, the diagnosing rate (46.3%) was lower in jointed examinations of biopsy and brush, and brush diagnosing rate (43.9%) was higher than biopsy (26.8%). Whether only biopsy diagnosing rate (35.8%) or only brush diagnosing rate (57.8%) on all cases of lung cancer, they were lower than jointed diagnosing rate (76.2%)of biopsy and brush. Examination fo CEA, sIgA, SF and β_2—MG in PB and BALF from lung cancer patients, it was findings that examinations of SF and β_2—MG in PB and BALF were no value in diagnosing lung cancer (P>0.05), but examinations of CEA in PB, CEA and sIgA difference value in tumor—bearing side nontumor—bearing side of BALF were helpful to diagnose and distinguish lung cancer. Synthetic examination of CEA and sIgA in BALF from tumor—bearing side can enhance diagnostic sensitivity and specificity, specially for the patients in whom the FB biopsy and brush were negative.
This study showed that the diagnosing rate of FB biopsy and brush was 76.2%, but if it was combined with examinations of CEA, sIgA in BALF, the diagnosing rate rose to 98.1%. So, combining FB examinations with tumor marks in PB and BALF can increase lung cancer diagnosing rate.
