摘要
目的 :探讨特发性血小板减少性紫癜 (ITP)患儿的细胞免疫与体液免疫功能的变化。方法 :用流式细胞术 (FCM )与免疫散射速率比浊法分别测定 2 0例ITP患儿的淋巴细胞亚群的分布、免疫球蛋白 (IgG ,IgA和IgM)和补体 (C3 ,C4)的水平。结果 :在治疗前 ,ITP患儿的CD3 + ,CD4+ 和CD4+ /CD8+ 比值、NK细胞均下降 ,CD8+ 升高 ,免疫球蛋白亦降低 ,对照组及恢复组比较有显著性差异 (P <0 .0 5 ) ,而B淋巴细胞 (CD9+)、补体(C3 ,C4)没有明显的变化 ;在治疗后 ,血小板恢复正常 (>10 0× 10 9/L)时 ,ITP患儿的CD3 + ,CD4+ 和CD4+ /CD8+ 比值、NK细胞、免疫球蛋白均回升至正常 ,CD8+ 下降 ,与治疗前比较有显著性差异 (P <0 .0 5 )。结论 :ITP患儿存在细胞免疫与体液免疫的紊乱 ,在治疗上应着眼纠正免疫紊乱。
Objective:To investigat the changes in cellular and humoral immunity in children with idiopathic thrombocytopenic purpura (ITP).Methods:The T lymphocyte subgroup, immunoglobulin and complement of 20 children with ITP were determined by flow cytometry and immunonephelometry respectively.Results:Compared with the normal control and remission group, these children with ITP showed an elevation in CD 8 + cells, but decrease in CD 3 + cells, CD 4 + cells, NK cells, ratio of CD 4 +/CD 8 + as well as immunoglobulin before treatment ( P <0.01~0.05),and the B lymphocyte and complement don't change significantly; while the thrombocyte in peripheral blood was normal level (>100×10 9/L) after treatment, the CD 3 + cells, CD 4 + cells, NK cells, ratio of CD 4 +/CD 8 + and immunoglobulin were significantly increased and CD 8 + cells were declined ( P <0.01~0.05).Conclusion:There were disorder in cellular and humoral immunity of children with ITP, and it was helpful for clinical diagnosis and treatment.
