摘要
应用单克隆抗体检测20例小儿肾母细胞瘤患者外周血T淋巴细胞亚群。结果显示:患者CD^+_3、CD^+_8均明显降低,CD^+_4有下降趋势,CD^+_4/CD^+_8比率降低。此外,动态观察了12例患者手术前后T淋巴细胞亚群变化,发现术后CD^+_3、CD^+_4变化不大,CD^+_8明显增高,长期随访患者,术后一年各项指标已恢复至接近正常小儿水平,而治疗中在于肿瘤复发或转移的患者,治疗前各项指标均处于十分低下状态,治疗后上述指标不但不回升后而继续下降。重点讨论患者CD^+_8不升高反而降低这一特殊现象的可能原因及T细胞亚群变化在评估疗效,提示预后方面的意义。
Peripheral blood T lymphocyte subsets were quantitated by monoclonal antibodies in 20 patients with Wilms' tumors. The total number of lymphocytes (CD3+)and T suppressor cells/killer cells (CD8+) decreased signifiantly. T helper cells (GD4+) tcndedeto decrease. Statistically significant decrease in the CD4+/CD8+ ratio was shown also. In addition, T-lymphocyte subsets were also studied before and after operation in 12 patients with Wilms' tumor. The results showed no change in CD3+ and CD4+, but significant increase in CD8+. The patients who had received a long-term observation had recovered to the level of mor-n mal children for CD3+, CD4+ and CD8+ one year after operation. However, those who died from recurrence or metastasis of the tumors had a very low level before treatment and couldn't increase but decrease after treatment for above criteria. We discussed with emphy-sis on the possible cause of CD8+ decrease and the significance of the changes of T lymphoc-cyte subsets in evaluation of the therapemtic effect and estimation of the prognosis.
