摘要
目的探讨点阵CO2激光治疗痤疮凹陷性瘢痕的疗效及安全性。方法以Dreno瘢痕性质作为诊断标准纳入83例患者,采用10600 nm CO2激光进行治疗,选用点阵、脉冲模式,点距1.1,横距8.8,纵距8.8,能量30~40 m J,扫描2遍,穿透深度450~600μm,治疗1次为一疗程。术后局部外用1周重组牛碱性成纤维细胞生长因子凝胶以促进修复。根据ECCA评分表评分及计算总分,计算疗效指数,并按基本治愈、显效、有效、无效进行疗效判定。结果完成1、3、6个月随访的患者分别为:83例(100%)、77例(92.8%)、65例(78.3%)。ECCA总分数治疗前为(62.5±4.6)分;治疗后1个月为(50.1±6.7)分;治疗后3个月为(35.5±7.9)分;治疗后6个月为(36.5±8.5)分。治疗后1、3、6个月ECCA总分低于治疗前(P<0.05);治疗后3、6个月ECCA总分低于治疗后1个月(P<0.05);治疗后3个月与治疗后6个月相比,其差异无统计学意义。所有患者不良反应均为暂时的且可耐受。结论采用点阵CO2激光治疗痤疮凹陷性瘢痕疗效确切,安全性好,值得临床推广应用。
Objective To explore the efficacy and safety of fractional CO2 laser in the treatment of atrophic acne scar. Methods Eighty-three patients were enrolled in this study according to the criteria of the ECCA(Dreno et al) scale. Fractional CO2 laser of10600 nm was adopted. Fractional and pulsed mode was selected, with a point distance of 1.1, horizontal distance of 8.8, longitudinal distance of 8.8, and energy of 30 to 40 m J. All patients were scanned 2 times with a penetration depth of 450 to 600 μm. A course consisted of one treatment. Recombinant bovine basic fibroblast growth factor(rb-bFGF) gel was locally externally applied for 1 week after the laser treatment. The total score and efficacy index were calculated according to the ECCA grading scale and assessed according to a scale of: basically cured, obviously effective, effective and ineffective. Results Among all patients, 83 cases(100%) completed follow-up for 1 month,77 cases(92.8%) completed follow-up for 3 months, and 65 cases(78.3%) completed follow-up for 6 months. The total ECCA scores were62.5 ±4.6 pre-treatment, 50.1 ±6.7 at 1 month post-treatment, 35.5 ±7.9 at 3 months post-treatment, and 36.5 ±8.5 at 6 months post-treatment. The condition at 1 month, 3 months, and 6 months post-treatment was better than that of pre-treatment(P<0.05). The efficacy at 3 months and 6 months post-treatment was better than that measured at 1 month post-treatment(P<0.05). There was no difference between the efficacy at 3 and 6 months post-treatment. All adverse effects were temporary and endurable. Conclusion Fractional CO2 laser was effective and safe in the treatment of atrophic acne scar. It is recommended for clinical popularization and application.
