摘要
目的运用Meta方法分析温阳益气利水活血方剂联合西医方案治疗慢性心力衰竭的疗效。方法计算机配合手工检索国内外常用数据库,筛选出符合纳入标准的相关文献,进行文献质量评估,采用Rev Man 5.3软件进行Meta分析。结果 12篇随机对照临床试验文献纳入研究,其中11篇中文文献和1篇外文文献,共计1 091例患者,其中对照组518例,观察组573例。Meta分析结果表明,中药观察组在中医证型表现、NYHA心功能分级、左室射血分数、血浆pro-BNP等疗效指标方面,均优于西医对照组,且有统计学意义。结论温阳益气利水活血方剂联合西医疗法治疗慢性心力衰竭有显著性疗效,但鉴于纳入研究数目较少,质量评价多有不足,存在发表偏倚可能性,故应客观谨慎看待结论,期待更佳临床试验参与验证。
Objective To compare the efficacy of warming-yang, tonifying-qi, excreting-water, invigorating-blood recipe combined with western medicine in the treatment of chronic heart failure by Meta analysis. Methods The computer associating with manual method was used to search the common databases at home and abroad, to screen out the relevant documents that met the inclusion criteria, to evaluate the literature quality, to use RevMan 5.3 software for Meta analysis. Results Finally, 12 randomized controlled clinical trial literatures were included in the study, of which 11 were Chinese and 1 was foreign language. A total of 1 091 patients, including 518 patients in the control group and 573 patients in the observation group, were included in the study. The results of Meta analysis showed that the Chinese medicine observation group was superior to the western medicine control group in TCM syndromes, NYHA cardiac function grading, left ventricular ejection fraction, plasma pro-BNP and so on, and had statistical significance. Conclusion There is an obvious curative effect of warming-yang, tonifying-qi, excreting-water, invigorating-blood recipe combined with western medicine in the treatment of chronic heart failure. In view of the small number of studies, the quality evaluation is insufficient, and there is a possibility of publication bias. Therefore, conclusions should be viewed objectively and carefully, and better clinical trials should be expected to participate in validation.
