高压氧治疗急性缺血性脑卒中的介入时机与疗效分析

摘要 目的分析高压氧治疗急性缺血性脑卒中的介入时机,观察临床治疗效果。方法选取所在医院于2016年7月-2018年9月收治的88例急性缺血性脑卒中患者,均接受高压氧治疗,根据治疗时机分组,观察组(发病第1 d内行高压氧治疗,n=44),对照组(发病3 d后行高压氧治疗,n=44),对比两组患者临床效果。结果观察组和对照组总有效率(90.91%vs 88.64%)对比差异无统计学意义(χ2=0.124,P>0.05);观察组治疗4周TNF-α、NPY、ET值均优于对照组,差异有统计学意义(P<0.05);观察组治疗2周、4周后NIHSS评分均低于对照组,差异有统计学意义(P<0.05)。结论高压氧治疗方法临床效果明显,但是介入治疗时间越早,患者神经功能、炎性因子指标改善越明显。 Objective To analyze the intervention time of hyperbaric oxygen in the treatment of acute ischemic stroke,and to observe the clinical therapeutic effect. Methods A total of 88 patients with acute ischemic stroke admitted to our hospital from July 2016 to September 2018 were selected. According to the intervention time, they were divided into observation group(hyperbaric oxygen treatment was performed within the first day of onset, n=44) and control group(hyperbaric oxygen therapy was performed 3 d after onset, n=44). The clinical effects of the two groups were compared.Results There was no statistically significant difference in the total effective rate between the observation group(90.91%) and the control group(88.64%)(χ2=0.124,P>0.05). After the treatment for four weeks, the TNF-a, NPY and ET values of the observation group were better than those of the control group,the different was statistically siginificant(P <0.05). The NIHSS scores of observation group were lower than those of the control group after 2 and 4 weeks of treatment,the different was statistically siginificant(P<0.05). Conclusion The clinical effect of hyperbaric oxygen therapy is obvious. And the earlier the interventional therapy implements, the more obvious the improvement of neurological function and inflammatory factor index is.
